GENERAL CONSTRUCTION NOTES:
1.ALL PUBLIC IMPROVEMENTS SHALL BE CONSTRUCTED PER THE APPLICABLE
SECTIONS OF THE CITY OF TIGARD PUBLIC IMPROVEMENT DESIGN STANDARDS,
EXCEPT AS MODIFIED HEREIN, THE 2018 APWA OREGON STANDARD
SPECIFICATIONS FOR CONSTRUCTION AND CLEAN WATER SERVICES
DISTRICT'S DESIGN AND CONSTRUCTION STANDARDS (RESOLUTION AND
ORDER 17-05). THE CONTRACTOR MUST COMPLY WITH ALL APPLICABLE
PROVISIONS OF FEDERAL AND STATE LAW AND THE TIGARD MUNICIPAL CODE.
2.EXISTING UTILITY LOCATIONS ARE APPROXIMATE ONLY. IN ORDER TO PROTECT
EXISTING UNDERGROUND UTILITIES, CONTRACTORS PERFORMING WORK
SHOWN ON THESE PLANS MUST NOTIFY UTILITIES AND PUBLIC AGENCIES AT
LEAST 48 BUSINESS HOURS IN ADVANCE OF, AND NO MORE THAN 10 BUSINESS
DAYS BEFORE, BEGINNING EXCAVATION, IN ACCORDANCE WITH THE
PROVISIONS OF OAR 952-001-0090. LIMITS OF WORK SHALL BE PRE-MARKED
FOR THE UTILITY LOCATORS. POTHOLE ALL CROSSINGS AS NECESSARY TO
PREVENT GRADE AND ALIGNMENT CONFLICTS. REPORT ALL CONFLICTS TO THE
ENGINEER IMMEDIATELY. PROTECT EXISTING UTILITIES AT ALL TIMES DURING
CONSTRUCTION. CALL THE ONE CALL UTILITY NOTIFICATION CENTER AT
503-246-6699 FOR UTILITY LOCATES. ANY DAMAGE TO EXISTING UTILITIES,
WHETHER THEY'RE SHOWN ON THESE DRAWINGS OR NOT, WILL BE REPAIRED
OR REPLACED AT THE CONTRACTOR'S EXPENSE. ATTENTION: OREGON LAW
REQUIRES ALL EXCAVATORS TO FOLLOW RULES ADOPTED BY THE OREGON
UTILITY NOTIFICATION CENTER. THOSE RULES ARE SET FORTH IN OAR
952-001-0010 THROUGH OAR 952-001-0090. YOU MAY OBTAIN COPIES OF THE
RULES BY CALLING THE CENTER. (NOTE: THE TELEPHONE NUMBER OF THE
ADMINISTRATION OFFICE FOR THE OREGON UTILITY NOTIFICATION CENTER IS
503-232-1987).
3.PRIVATE FACILITIES MAY REQUIRE ADJUSTMENT OR RELOCATION. THE
CONTRACTOR SHALL COORDINATE WITH EACH AFFECTED UTILITY A MINIMUM
OF TWO WEEKS PRIOR TO WORK IN AN AREA REQUIRING ADJUSTMENT OR
RELOCATION OF A PRIVATE UTILITY'S FACILITY. SEE SPECIFICATION SECTION
00150.50(B).
4.THE CONTRACTOR SHALL EXERCISE ALL DUE CARE IN PROTECTING PROPERTY
ALONG THE ROUTE OF THE IMPROVEMENTS. THIS PROTECTION SHALL INCLUDE,
BUT NOT BE LIMITED TO, TREES, YARDS, FENCES, DRAINAGE LINES, MAIL
BOXES, DRIVEWAYS, SHRUBS, LAWNS, IRRIGATION SYSTEMS, WITHIN ANY
RIGHTS-OF-WAYS AND EASEMENTS. IF ANY OF THE ABOVE HAVE BEEN
DISTURBED, THEY SHALL BE RESTORED AS NECESSARY TO AS NEAR THEIR
ORIGINAL CONDITION AS POSSIBLE OR REPLACED IN KIND.
5.THE CONTRACTOR SHALL PERFORM ALL THE WORK SHOWN ON THE DRAWINGS
AND ALL INCIDENTAL WORK CONSIDERED NECESSARY TO COMPLETE THE
PROJECT IN AN ACCEPTABLE MANNER.
6.THE CONTRACTOR AND/OR EACH SUB-CONTRACTOR SHALL HAVE A MINIMUM
OF ONE SET OF CITY-APPROVED CONSTRUCTION PLANS ON THE JOB SITE AT
ALL TIMES WHILE WORK IS BEING DONE.
7.UPON COMPLETION OF CONSTRUCTION, THE CONTRACTOR SHALL SUBMIT
"REDLINE DRAWINGS" TO PROJECT ENGINEER FOR PREPARATION OF RECORD
DRAWINGS. "REDLINE DRAWINGS" DOCUMENT ALL DEVIATIONS AND REVISIONS
TO THE APPROVED PLANS; THEY ALSO RECORD A DESCRIPTION OF
CONSTRUCTION MATERIALS ACTUALLY USED (PIPE MATERIAL, ETC.)
8.A COPY OF THE PERMIT AND ALL ATTACHMENTS, AND A COPY OF THE
APPROVED CONSTRUCTION PLAN AND ALL AMENDMENTS SHALL BE AVAILABLE
AT THE WORK AREA. ALL WORK SHALL CONFORM TO THE PERMIT TERMS,
CONDITIONS AND PROVISIONS AND TO THE CITY APPROVED PERMIT PLANS,
AND APPROVED PLAN AMENDMENTS AND TO THE CITY'S STANDARDS AND
SPECIFICATIONS AND TO THESE GENERAL CONDITIONS. CHANGES TO ANY OF
THE AFORESAID MUST BE APPROVED BY THE CITY, IN ADVANCE OF WORK
PERFORMANCE.
9.IT IS THE CONTRACTOR'S RESPONSIBILITY TO VISIT THE SITE AND POTHOLE AS
NEEDED TO VERIFY ALL EXISTING CONDITIONS BEFORE THE START OF WORK.
THE CONTRACTOR SHALL TAKE ALL NECESSARY FIELD MEASUREMENTS AND
OTHERWISE VERIFY ALL DIMENSIONS AND EXISTING CONSTRUCTION
CONDITIONS INDICATED AND/OR SHOWN ON THE PLANS. SHOULD ANY ERROR
OR INCONSISTENCY EXIST, THE CONTRACTOR SHALL NOT PROCEED WITH THE
WORK AFFECTED UNTIL REPORTED TO THE PROJECT ENGINEER FOR
CLARIFICATION OR CORRECTION.
10.THE CONTRACTOR SHALL MAKE PROVISIONS TO KEEP ALL EXISTING UTILITIES
IN SERVICE AND PROTECT THEM DURING CONSTRUCTION. CONTRACTOR
SHALL IMMEDIATELY REPAIR OR REPLACE ANY DAMAGED UTILITIES USING
MATERIAL AND METHODS APPROVED BY THE UTILITY OWNER. NO SERVICE
INTERRUPTIONS SHALL BE PERMITTED WITHOUT PRIOR WRITTEN APPROVAL
FROM THE UTILITY PROVIDER.
11.THE CONTRACTOR SHALL PRUNE ALL VEGETATION, AS NECESSARY AWAY AND
UP FROM THE STREET AND SIDEWALK. AS WELL AS ANY ROOT PRUNING AS
DETERMINED BY THE ENGINEER. THE CONTRACTOR SHALL PROTECT ALL
EXISTING LANDSCAPING THAT IS TO REMAIN.
12.AT THE END OF EACH WORK DAY, THE CONTRACTOR SHALL CLEAN UP THE
PROJECT AREA AND LEAVE IT IN A NEAT AND SECURED MANNER. UPON
COMPLETION, THE CONTRACTOR SHALL LEAVE THE PROJECT AREA FREE OF
DEBRIS AND UNUSED MATERIAL.
13.THE CONTRACTOR SHALL MAINTAIN AND COORDINATE ACCESS TO ALL
PROPERTIES/BUSINESSES ADJACENT TO THE PROJECT. THE CONTRACTOR
SHALL NOTIFY AFFECTED PROPERTY/ BUSINESS OWNERS A MINIMUM OF 48
HOURS PRIOR TO ANY DRIVEWAY CLOSURES AND ASSIST PROPERTY OWNER
WITH ALTERNATIVE PARKING. PEDESTRIAN AND VEHICLE ACCESS TO
ENTRANCES SHALL BE MAINTAINED AT ALL TIMES. THE CONTRACTOR SHALL
PROVIDE RAMPS OR OTHER APPROVED METHODS FOR MAINTAINING ACCESS
TO ENTRANCES. RAMPS SHALL BE ADA COMPLIANT, APPROVED BY THE
ENGINEER, AND INCIDENTAL TO THE CONTRACT WORK.
14.THE CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR COMPLYING WITH ALL
CONSTRUCTION SAFETY, HEALTH AND OTHER RULES AND REGULATIONS FROM
OSHA, DEQ, STATE, AND LOCAL REGULATING AGENCIES FOR SAFETY AND
INSTALLATION OF THE WORK INCLUDING BUT NOT LIMITED TO SHORING,
BRACING, ERECTION/INSTALLATION, FALL PROTECTION, GUARDRAILS, ETC.
15.IF CULTURAL OR ARCHAEOLOGICAL RESOURCES ARE DISCOVERED ON THE
SITE DURING CONSTRUCTION ACTIVITY, INCLUDING BURIAL SITES, THE CITY OF
TIGARD SHALL BE NOTIFIED IMMEDIATELY AND THE CONTRACTOR SHALL
COMPLY WITH ALL REQUIREMENTS OF SECTION 00290.50 OR 00290.51 OF THE
OREGON STANDARD SPECIFICATIONS AS APPLICABLE.
16.MATERIALS SHALL BE NEW. THE USE OF MANUFACTURER'S NAMES, MODELS,
AND NUMBERS IS REQUIRED AND INTENDED TO ESTABLISH STYLE, QUALITY,
APPEARANCE, AND USEFULNESS. PROPOSED SUBSTITUTIONS WILL REQUIRE
WRITTEN APPROVAL FROM THE CITY OF TIGARD PRIOR TO INSTALLATION.
17.BEFORE INITIATING ANY CONSTRUCTION ACTIVITY, THE CONTRACTOR SHALL
CONTACT THE CITY'S PROJECT MANAGER, ANDREW NEWBURY (503-718-2472)
TO ESTABLISH A PLACE, TIME, AND DATE FOR A PRE-CONSTRUCTION MEETING.
18.THE CONTRACTOR SHALL NOTIFY THE CITY PROJECT MANAGER, ANDREW
NEWBURY (503-718-2472) 24 HOURS PRIOR TO COMMENCING WORK, 24 HOURS
PRIOR TO ANY STAGED INSPECTION, AND AFTER COMPLETING WORK COVERED
BY THE PERMIT.
19.MAINTENANCE OF THE WORK AREA AND APPROACH ROADS IS THE
RESPONSIBILITY OF THE CONTRACTOR. THE WORK AREA AND APPROACH
ROADS SHALL BE MAINTAINED IN A CLEAN CONDITION, FREE FROM
OBSTRUCTIONS AND HAZARDS. A COPY OF THE CONTRACTORS CERTIFICATE
OF INSURANCE SHALL BE AVAILABLE AT THE WORK AREA.
20.THE SPREADING OF MUD OR DEBRIS OR STORAGE OF MATERIALS OR
EQUIPMENT OF ANY KIND UPON ANY PUBLIC ROADWAY IS STRICTLY
PROHIBITED AND VIOLATION SHALL BE CAUSE FOR IMMEDIATE CANCELLATION
OF THE PERMIT. THE CITY MAY AT ANY TIME ORDER IMMEDIATE CLEAN UP AND
STOPPAGE OF WORK TO ACCOMPLISH CLEAN UP.
21.EFFECTIVE EROSION CONTROL IS REQUIRED. EROSION CONTROL DEVICES
MUST BE INSTALLED AND MAINTAINED MEETING THE D.E.Q. REQUIREMENTS.
THE CITY MAY AT ANY TIME ORDER CORRECTIVE ACTION AND STOPPAGE OF
WORK TO ACCOMPLISH EFFECTIVE EROSION CONTROL.
22.PROPERTY DISTURBED BY CONSTRUCTION ACTIVITY SHALL BE SEEDED WITH A
STANDARD GRASS MIX; SHRUBS, FLOWERS, BARKDUST, EXISTING SIGNS,
PAVEMENT MARKINGS, MAILBOXES, ETC. SHALL BE REESTABLISHED,
REINSTALLED OR REPLACED, WITH LIKE KIND AND MATERIAL.
23.EFFECTIVE DRAINAGE CONTROL IS REQUIRED. DRAINAGE SHALL BE
CONTROLLED WITHIN THE WORK SITE AND SHALL BE SO ROUTED THAT
ADJACENT PRIVATE PROPERTY, PUBLIC PROPERTY AND THE RECEIVING
SYSTEM IS NOT ADVERSELY IMPACTED. THE CITY MAY AT ANY TIME ORDER
CORRECTIVE ACTION AND STOPPAGE OF WORK TO ACCOMPLISH EFFECTIVE
DRAINAGE CONTROL.
24.CONTRACTOR MUST VERIFY ALL EXISTING UTILITIES FOR BOTH VERTICAL
ELEVATION AND HORIZONTAL LOCATION PRIOR TO START OF WORK (POTHOLE
BEFORE DIGGING IF NECESSARY). SHOULD CONFLICTS ARISE AND REDESIGN
OR RELOCATION OF FACILITIES BE NECESSARY, IT SHALL BE DONE AT THE
CONTRACTORS EXPENSE. CHANGES MUST BE APPROVED BY THE CITY IN
ADVANCE OF WORK PERFORMANCE. CONTRACTOR SHALL COORDINATE THE
WORK WITH AFFECTED UTILITY AGENCIES.
25.A TEMPORARY HARD-SURFACE PATCH (COLD MIX AC OR HOT MIX BASE PAVING)
SHALL BE PLACED ON TRENCHES WITHIN ROADWAYS AT THE END OF EACH
DAYS WORK. NO TRENCH, ON SITE OR OFF-SITE, SHALL BE LEFT AT ANY TIME IN
AN UNSAFE CONDITION. THE CONTRACTOR IS RESPONSIBLE FOR AND IS LIABLE
FOR HAZARDS OR DAMAGE RESULTING FROM THE PROSECUTION OF THE
WORK.
26.WORK PROVIDED FOR UNDER THE PERMIT SHALL INCLUDE REPAIR OF EXISTING
FACILITIES (ROADS, DITCHES, ETC.) AS MAY BE NECESSARY, IN THE CITY
INSPECTORS OPINION, TO OVERCOME DETERIORATION OR DAMAGE WHICH
OCCURRED IN CONJUNCTION WITH THE WORK AUTHORIZED BY THE PERMIT.
CORRECTIVE WORK SHALL BE DONE AT THE CONTRACTORS' EXPENSE.
27.THE CITY'S INSPECTORS MAY, AT THEIR DISCRETION, REQUIRE PROVISION OF
TESTS AND OR REPORTS FROM THE CONTRACTOR TO VALIDATE CLAIMS OF
MATERIAL OR CONSTRUCTION ADEQUACY/ COMPLIANCE. SUCH
TESTS/REPORTS SHALL BE PROVIDED AT THE CONTRACTORS' EXPENSE.
28.EXISTING MONUMENTS, PROPERTY CORNERS, AND SURVEY MARKERS SHALL
BE PROTECTED. REPLACEMENT SHALL BE AT THE CONTRACTORS' EXPENSE.
29.THE CONTRACTOR SHALL PROVIDE TO THE CITY INSPECTOR, IN WRITING, THE
NAMES AND 24 HOUR EMERGENCY TELEPHONE NUMBER OF TWO (2) PERSONS
WHO HAVE AUTHORITY TO RESOLVE PROBLEMS, TAKE CORRECTIVE ACTION
AND, IN GENERAL, WILL BE RESPONSIBLE IN CASE OF ANY EMERGENCY. THE
CONTRACTOR SHALL NOTIFY THE CITY INSPECTOR, IN WRITING, OF ANY/ALL
ASSIGNMENT CHANGES.
30.THE CONTRACTOR SHALL PROVIDE TO THE CITY INSPECTOR, IN WRITING, THE
NAME AND 24 HOUR EMERGENCY TELEPHONE NUMBER OF A DESIGNATED
"COMPETENT PERSON" RESPONSIBLE FOR CONSTRUCTION SAFETY AS PER
OR-OSHA, CHAP. 437, DIV. 3 CONSTRUCTION, SUB-DIVISION P - EXCAVATIONS.
THE CONTRACTOR SHALL NOTIFY THE CITY INSPECTOR OF ANY/ALL
ASSIGNMENT CHANGES.
31.OREGON LAW REQUIRES FOLLOWING THE RULES ADOPTED BY THE OREGON
UTILITY NOTIFICATION CENTER. SAID RULES ARE SET FORTH IN OAR
952-001-0010 THROUGH OAR 952-001-0080. COPIES OF SAID RULES MAY BE
OBTAINED FROM THE CENTER BY CALLING 503-246-1987. IF YOU HAVE ANY
QUESTION ABOUT THE RULES, CONTACT THE CENTER. NOTE: DAMAGE TO
UTILITIES SHALL BE CORRECTED AT THE PERMIT HOLDERS EXPENSE.
32.IT IS THE SOLE RESPONSIBILITY OF THE PERMIT HOLDER TO PROVIDE FOR
PROPER RIGHT-OF-ENTRY AND/OR EASEMENTS PRIOR TO STARTING WORK.
PROOF OF RIGHT-OF-ENTRY OR PROPERLY EXECUTED EASEMENTS, SHALL BE
PROVIDED TO THE CITY. THE CITY SHALL IN NO WAY BE CONSTRUED TO BE
LIABLE FOR THE PERMIT HOLDERS FAILURE TO OBTAIN OR PROVIDE FOR
PROOF OF RIGHT-OF-ENTRY OR EASEMENTS.
33.CONTRACTOR TO SUBMIT A DEWATERING PLAN FOR APPROVAL PRIOR TO
EXCAVATION
STREETS AND STRUCTURAL FILLS NOTES:
1.ALL TREES, BRUSH AND DEBRIS WITHIN THE LIMITS OF THE RIGHT-OF-WAY AND
ON THE AREAS TO BE FILLED SHALL BE REMOVED AND DISPOSED OF BY THE
CONTRACTOR UNLESS OTHERWISE NOTED ON THE PLANS OR FLAGGED IN THE
FIELD.
2.ALL AREAS OF CONSTRUCTION SHALL BE STRIPPED. STRIPPING SHALL
CONSIST OF REMOVING THE TOPSOIL HUMUS. STRIPPING DEPTH SHALL BE 6"
PER 00220.41. STRIPPING MATERIALS SHALL BE PLACED OR STOCKPILED BY
THE CONTRACTOR ON SITE AS SHOWN ON THE PLANS AND PER INSTRUCTION
BY THE INSPECTOR, OR HAULED OFF SITE TO AN APPROVED LOCATION.
3.EMBANKMENTS AND STRUCTURAL FILLS FOR ROADWAY CONSTRUCTION OR
FILLS TO BE CONSTRUCTED ON BUILDABLE AREAS SHALL BE CONSTRUCTED
FROM EXCAVATED MATERIALS ACCEPTABLE TO THE SOILS ENGINEER AND
SHALL BE BROUGHT TO GRADE IN LIFTS NOT TO EXCEED 8†LOOSE MEASURE.
EACH LIFT SHALL BE COMPACTED TO 95 PERCENT OF MAXIMUM DENSITY AS
OBTAINED BY AASHTO T-99 COMPACTION TEST. COMBINATION TEST RESULTS
SHALL BE SUBMITTED TO THE CITY INSPECTOR.
4.FILLS SHALL NOT BE CONSTRUCTED ON NATURAL SLOPES STEEPER THAN 2
HORIZONTAL TO 1 VERTICAL. ALL FILL SLOPES SHALL NOT EXCEED 2
HORIZONTAL TO 1 VERTICAL. NO ROCK OR SIMILAR IRREDUCIBLE MATERIAL
WITH A MINIMUM DIMENSION GREATER THAN 12 INCHES SHALL BE BURIED OR
PLACED IN THE FILLS.
5.ROCK BASE FOR ASPHALTIC CONCRETE PAVEMENT, CURB AND SIDEWALK
CONSTRUCTION SHALL BE AS SHOWN ON THE TYPICAL SECTIONS AND DETAIL
SHEET AND IN ACCORDANCE WITH THE ABOVE REFERENCED SPECIFICATIONS.
6.THE CONTRACTOR SHALL CLEAN ALL SPILLED DIRT, GRAVEL OR OTHER

STREETS AND ROADS AT THE CONCLUSION OF EACH DAY OR OPERATION.
CLEANING SHALL BE BY GRADER AND FRONT-END LOADER, SUPPLEMENTED BY
POWER BRUSHING AND HAND LABOR UNLESS OTHERWISE APPROVED BY THE
CITY. THE CONTRACTOR SHALL FOLLOW CITY AND CWS EROSION CONTROL
PROCEDURES.
7.AS SOON AS PRACTICAL AFTER COMPLETION OF ALL PAVING AND GRAVEL
SHOULDER RESURFACING, THE CONTRACTOR SHALL REMOVE ALL DIRT, MUD,
ROCK GRAVEL AND OTHER FOREIGN MATERIAL FROM THE PAVED SURFACE
AND STORM DRAINAGE SYSTEM.
8.ALL RADII INDICATE FACE OF CURB OR EDGE OF PAVEMENT, AS APPLICABLE.
ALL DIMENSIONING IS WITH RESPECT TO FACE OF CURB.
9.ALL CUT EDGES SHALL BE SAND SEALED WITH CRS-1 OR CRS-2 EMULSIFIED
ASPHALT OR APPROVED EQUAL.
TRAFFIC CONTROL NOTES:
1.THE CITY'S STANDARD SPECIFICATION FOR TRAFFIC CONTROL IS "MANUAL ON
UNIFORM TRAFFIC CONTROL DEVICES FOR STREETS AND HIGHWAYS", U.S.
DEPT. OF TRANSPORTATION, FHWA, CURRENT EDITION.
2.TRAFFIC CONTROL SHALL BE PROVIDED FOR BY THE CONTRACTOR IN
ACCORDANCE WITH THE CITY'S STANDARD SPECIFICATIONS AND, ALSO, IN
ACCORDANCE WITH A CITY (JOB SPECIFIC) APPROVED TRAFFIC CONTROL PLAN.
A COPY OF THE APPROVED TRAFFIC CONTROL PLAN SHALL BE AVAILABLE AT
THE WORK AREA.
3.PUBLIC ROADWAY SHALL NOT BE CLOSED TO TRAFFIC, AT ANY TIME, WITHOUT
HAVING FIRST OBTAINED WRITTEN APPROVAL FROM THE CITY ENGINEER. THE
PERMIT HOLDER IS RESPONSIBLE FOR PROVISION OF TIMELY NOTIFICATION OF
TRAFFIC FLOW DISRUPTIONS TO AREA WIDE EMERGENCY SERVICES (TIGARD
POLICE DEPT., TUALATIN FIRE & RESCUE) AND TO TRI-MET AND TIGARD
SCHOOL DISTRICT AND OTHER SERVICE PROVIDERS.
4.ADVANCE WARNING OF IMMINENT TRAFFIC DISRUPTION SHALL BE PROVIDED
TO THE GENERAL MOTORING PUBLIC BY PLACEMENT OF AN ADVANCE
NOTIFICATION SIGN AT EACH END OF THE CONSTRUCTION AREA 72 HOURS
(MIN.) BEFORE INITIATION OF CONSTRUCTION WORK.
5.ACCESS TO EXISTING PROPERTIES SHALL BE MAINTAINED AT ALL TIMES,
INCLUDING NORMAL DELIVERY SERVICE AND MAIL SERVICE AND IF NOT, SHALL
BE CAUSE FOR WORK STOPPAGE UNTIL EFFECTIVE ACCESS IS ESTABLISHED.
6.TRAFFIC CONTROL DEVICES, FLAGPERSONS, ETC., SHALL BE IN PLACE PRIOR
TO INITIATION OF CONSTRUCTION WORK AND SHALL BE EFFECTIVELY
MAINTAINED.
7.PURSUANT TO TIGARD MUNICIPAL CODE SECTION 7.40.180, WORK IS
PERMITTED BETWEEN THE HOURS OF 7:00 A.M. TO 7:00 P.M. DAILY UNLESS
OTHERWISE AUTHORIZED BY THE CITY MANAGER.
8.MINIMUM TRAVEL LANE WIDTH SHALL BE TWELVE (12) FEET; PEDESTRIAN
TRAVEL SHALL ALSO BE PROVIDED FOR.
9.THE CITY RESERVES THE RIGHT TO ADD TO OR MODIFY TRAFFIC CONTROL
REQUIREMENTS AS MAY BE NECESSARY TO EFFECTIVELY CONTROL TRAFFIC
AND TO ASSURE PUBLIC SAFETY.
10.TRAFFIC CONTROL TO BE PERFORMED IN ACCORDANCE WITH THE MANUAL
FOR UNIFORM TRAFFIC CONTROL DEVICES AND OREGON AMENDMENTS AS
REQUIRED. THE CITY CAN REQUIRE ADDITIONAL TRAFFIC CONTROL MEASURES
AS NEEDED TO PROVIDE FOR PUBLIC SAFETY.
11.THE CONTRACTOR SHALL SUBMIT A TRAFFIC CONTROL PLAN FOR APPROVAL
PER THE SPECIFICATIONS 7 DAYS PRIOR TO STARTING CONSTRUCTION.
12.THE CONTRACTOR SHALL COORDINATE ACCESS TO DRIVEWAYS WITH
PROPERTY OWNERS. ACCESS TO ROADWAY APPROACHES WITHIN THE
PROJECT LIMITS SHALL BE MAINTAINED AT ALL TIMES. TRAFFIC PLATES SHALL
BE USED (AND SECURED IN A MANNER ACCEPTABLE BY THE AGENCY) ACROSS
ALL TRENCHES BLOCKING DRIVEWAYS TO PROVIDE ACCESS AT ALL TIMES. AT
NO TIME SHALL CONTRACTORS DETAIN OR DELAY EMERGENCY VEHICLES.
13.CONTRACTOR SHALL COORDINATE ACCESS FOR SERVICES INCLUDING, BUT
NOT LIMITED TO, MAIL DELIVERY, TRASH PICKUP, SCHOOL TRANSPORTATION,
AND ANY OTHER SPECIAL TRANSPORTATION SERVICES THAT EXIST WITHIN THE
PROJECT AREA.
14.EXISTING SIGNS THAT CONFLICT WITH CONSTRUCTION SIGNING ARE TO BE
COVERED OR REMOVED AND REINSTALLED AFTER CONSTRUCTION.
15.GRADE CHANGES WITHIN THE TRAVELED LANE OR A VERTICAL CHANGE OF
MORE THAN 1" WILL REQUIRE TEMPORARY HMAC RAMP WITH PAPER OR SAND
JOINTS AS NEEDED.
16.CONTRACTOR SHALL PROVIDE SAFE, STABLE, AND ACCESSIBLE ACCESS TO
ALL DRIVEWAY AND PEDESTRIAN PATHS CONNECTING TO FRONT DOORS AT
ALL TIMES. TEMPORARY INTERRUPTIONS IN ACCESS SHALL BE COORDINATED
BY THE CONTRACTOR WITH THE PROPERTY OWNER/RENTER AS REQUIRED.
17.MAINTENANCE OF THE WORK AREA AND APPROACH ROADS IS THE
RESPONSIBILITY OF THE PERMIT HOLDER. THE WORK AREA AND APPROACH
ROADS SHALL BE MAINTAINED IN A CLEAN CONDITION, FREE FROM
OBSTRUCTIONS AND HAZARDS. A COPY OF THE PERMIT HOLDERS
CERTIFICATE OF INSURANCE SHALL BE AVAILABLE AT THE WORK AREA.

PAVEMENT DEFICIENCIES. WORK MAY INCLUDE ADDITIONAL GRINDING AND
INLAY DEPTH, FULL REPLACEMENT OF PAVEMENT STRUCTURE INCLUDING
AGGREGATE BASE, OR OTHER IMPROVEMENTS AS DEEMED NECESSARY BY
THE CITY. CONTRACTOR SHALL SHOW APPROPRIATE TIME AND
RESOURCES IN THE PROJECT SCHEDULE TO COMPLETE THIS WORK.
10.PAVE DRIVEWAY AND STREET TRANSITIONS CONCURRENTLY WITH
MAINLINE PAVING UNLESS OTHERWISE APPROVED BY THE ENGINEER.
11.WHERE FULL PAVEMENT SECTION IS TO BE REPLACED TO MATCH EXISTING
GRADE AT THE CENTERLINE/ROADWAY CROWN AND ADJACENT TO THE
EXISTING CURB, CONTRACTOR SHALL:
·MARK EXISTING CURB FLOW LINE WITH PAINT PRIOR TO A.C.
SURFACING REMOVAL TO DOCUMENT EXISTING PAVEMENT ELEVATION
AND CURB EXPOSURE.
·SURVEY THE EXISTING ROADWAY CROWN, GRADE BREAKS, EDGE OF
PAVEMENT (AS NECESSARY) AND ANY OTHER LOCATIONS AS
NECESSARY TO RE-ESTABLISH THE EXISTING ROAD GRADE. EXISTING
ELEVATIONS SHALL BE COLLECTED AT A MAXIMUM DISTANCE OF 25
FEET ALONG THE STREET ALIGNMENT.
SURVEY INFORMATION SHALL BE USED TO PROVIDE CONSTRUCTION
STAKING, AS NECESSARY, DURING FINAL GRADING PRIOR TO PAVING.
12.IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO PROTECT THE
EXISTING SUBGRADE OF PROJECT SEGMENTS WHERE THE ENTIRE
THICKNESS OF EXISTING ASPHALT IS SCHEDULED FOR REMOVAL.
PROTECTION INCLUDES LIMITING HEAVY CONSTRUCTION EQUIPMENT

13.ALL SANITARY SEWER LINES WITHIN 10 FEET LATERALLY OR 3 FEET
VERTICALLY OF A WATER MAIN SHALL BE ENCASED IN A REINFORCED
CONCRETE JACKET 6 INCHES THICK FOR A DISTANCE OF 10 FEET ON BOTH
SIDES OF THE CROSSING. WHERE CROSSINGS ARE NECESSARY, THEY
MUST BE MADE AT APPROXIMATELY 90 DEGREES WITH AT LEAST 18 INCHES
OF SEPARATION BELOW THE WATER LINE.
14.THE CONTRACTOR SHALL SCHEDULE CONSTRUCTION WORK SPECIFIED
HEREIN TO MAINTAIN CONTINUOUS WATER SERVICE TO EXISTING WATER
USERS. WHERE IT IS NECESSARY TO SHUT DOWN SERVICE TO MAKE
REQUIRED INTERTIES, THE CONTRACTOR SHALL NOTIFY THE CITY
INSPECTOR AT LEAST FIVE (5) WORKING DAYS PRIOR TO A PLANNED
WATER SERVICE SHUT DOWN TO ALLOW THE OWNER TO NOTIFY USERS OF
THE UPCOMING TEMPORARY SUSPENSION OF WATER SERVICE.
15.PRIOR TO ANY CUT-IN OR CONNECTION, THE CONTRACTOR SHALL HOLD AN
ONSITE PRE-CONSTRUCTION MEETING WHERE THE CUT-IN SCHEDULE AND
COORDINATION WILL BE DISCUSSED. THOSE TO ATTEND SHALL INCLUDE
THE ONSITE FOREMAN, CITY PROJECT INSPECTOR, AND TIGARD WATER
PERSONNEL. THIS MEETING SHALL TAKE PLACE PRIOR TO EACH
CONNECTION AND BE HELD BETWEEN ONE WEEK PRIOR TO TWO (2)
WORKING DAYS PRIOR TO THE CONNECTION. THE CONTRACTOR SHALL
HAVE ALL FITTINGS, PIPE, CHLORINE SWABBING EQUIPMENT, PUMPS AND
HOSES, AND ALL EQUIPMENT NEEDED TO MAKE THE CUT-IN CONNECTION
AT THE MEETING AND AVAILABLE FOR CITY INSPECTION.
16.NEW PIPELINE SHALL BE FLUSHED, PRESSURE TESTED, AND DISINFECTED
BEFORE ANY CONNECTION TO THE EXISTING WATER SYSTEM IS MADE.
TEMPORARY BLOW-OFFS SHALL BE PROVIDED BY THE CONTRACTOR AT
ALL DEAD-ENDS. ALL FLUSHING, PRESSURE TESTING, AND DISINFECTION
WATER SHALL BE DELIVERED THROUGH AN APPROPRIATELY SIZED
OREGON HEALTH AUTHORITY (OHA) APPROVED DOUBLE CHECK VALVE
ASSEMBLY (DCVA) BACKFLOW PREVENTION DEVICE. THE CONTRACTOR
SHALL PROVIDE OR OBTAIN A DVCA BACKFLOW PREVENTION DEVICE. ONCE
INSTALLED, A CERTIFIED BACKFLOW TESTER SHALL TEST THE DEVICE AND
DOCUMENTATION MUST BE SUPPLIED TO THE CITY INSPECTOR BEFORE
ANY FLUSHING ACTIVITIES MAY BEGIN.
17.ALL PIPES, VALVES, AND FITTINGS SHALL BE THOROUGHLY FLUSHED PRIOR
TO PRESSURE TESTING AND CHLORINATION. FLUSHING SHALL BE DONE
THROUGH BLOW-OFF UNITS, HYDRANTS, INDIVIDUAL SERVICES, AND MAINS
AT A VELOCITY OF AT LEAST 2.5 FEET PER SECOND (FPS).
18.PRIOR TO ANY FLUSHING PROCEDURES, THE CONTRACTOR SHALL ISSUE A
FLUSHING PLAN PROVIDING DIRECTION OF FLOW, SURFACE WATER
MANAGEMENT BEST MANAGEMENT PRACTICES (BMPS), AND A WRITTEN
SCHEDULE TO THE CITY PROJECT INSPECTOR FOR APPROVAL. A TWO (2)
WORKING DAY NOTICE SHALL BE GIVEN TO THE CITY PROJECT INSPECTOR
PRIOR TO ANY SYSTEM SHUT-DOWN OR FLUSHING PROCEDURES. UNDER
NO CIRCUMSTANCES SHALL THE CONTRACTOR OPERATE ANY CITY
VALVES.
19.PRESSURE TESTING SHALL COMMENCE ONLY AFTER BACKFILL HAS
PASSED THE REQUIRED COMPACTION TESTS PER AASHTO T-180 AND THE
ROADWAY BASE ROCK HAS BEEN PLACED, COMPACTED, PROOF ROLLED,
AND APPROVED BY THE CITY PROJECT INSPECTOR.
20.ALL MAINS WITH A STATIC PRESSURE UP TO 100 POUNDS PER SQUARE
INCH (PSI) SHALL BE TESTED AT 150 PSI FOR 1 HOUR WITH A MAXIMUM
ALLOWABLE LOSS OF 5 PSI. WATER MAINS WITH A STATIC PRESSURE
GREATER THAN 100 PSI SHALL BE PRESSURE TESTED AT 1.5 TIMES THE
STATIC PRESSURE FOR 1 HOUR WITH A MAXIMUM LOSS OF 5 PSI. ALL
PRESSURE TESTING SHALL BE COMPLETED IN THE PRESENCE OF THE CITY
PROJECT INSPECTOR.
21.UPON SATISFACTORY COMPLETION OF PRESSURE TESTING, THE NEW
MAINS SHALL BE CLEANED AND FLUSHED WITH POTABLE WATER PRIOR TO
DISINFECTION. FLUSHING VELOCITIES SHALL BE AT LEAST 2.5 FPS. THE
PIPELINE SHALL BE THOROUGHLY FLUSHED, CHLORINATED, AND TESTED IN
ACCORDANCE WITH AWWA STANDARD C651-92, THE STATE HEALTH
DIVISION AND CITY REQUIREMENTS. THE CONTINUOUS FEED METHOD OF
DISINFECTION SHALL BE USED. DISINFECTING MIXTURE SHALL BE A
CHLORINE-WATER SOLUTION HAVING A FREE CHLORINE RESIDUAL OF 40
TO 50 MILLIGRAMS PER LITER (MG/L), AND IT SHALL BE INTRODUCED INTO
THE MAIN THROUGH A TAP NO MORE THAN 18 INCHES FROM THE
CONNECTION POINT TO THE EXISTING SYSTEM. THE DISINFECTION
MIXTURE WILL BE PREPARED BY INJECTING A CALCIUM OR SODIUM
HYPOCHLORITE AND WATER SOLUTION INTO THE PIPELINE AT A MEASURED
RATE WHILE FRESH, POTABLE WATER IS ALLOWED TO FLOW THROUGH THE
PIPELINE SO THAT THE CHLORINE WATER SOLUTION IS OF THE SPECIFIED
STRENGTH. TREATED (CHLORINATED) WATER SHALL BE RETAINED IN THE
PIPELINE LONG ENOUGH TO DESTROY ALL NON-SPORE-FORMING
BACTERIA. DURING THE CHLORINATION PROCESS, ALL VALVES AND
ACCESSORIES SHALL BE OPERATED. ALL PARTS OF THE LINES AND
SERVICES SHALL BE DISINFECTED. TYPICAL RETENTION PERIOD IS 24
HOURS. AT THE END OF THE 24-HOUR PERIOD, THE PIPELINE IS TO HAVE A
FREE CHLORINE RESIDUAL OF AT LEAST 10 MG/L.
AFTER SATISFACTORY DISINFECTION, FLUSH THE CHLORINATION WATER
FROM THE LINE UNTIL THE WATER THROUGHOUT THE PIPELINE IS EQUAL
CHEMICALLY AND BACTERIOLOGICALLY TO THE PERMANENT SOURCE OF
SUPPLY. THE DISINFECTION WATER MUST BE DISPOSED OF IN AN
APPROVED MANNER. DO NOT ALLOW DISINFECTION WATER TO FLOW INTO
A WATERWAY WITHOUT ADEQUATE DILUTION OR OTHER SATISFACTORY
METHODS OF REDUCING CHLORINE RESIDUALS TO A SAFE LEVEL AS
MANDATED BY THE DEPARTMENT OF ENVIRONMENTAL QUALITY (DEQ).
AFTER FLUSHING AND DISPOSAL OF THE DISINFECTION SOLUTION THERE
WILL BE ANOTHER 24-HOUR PERIOD PRIOR TO BACTERIOLOGICAL TESTING.
BACTERIOLOGICAL TEST SAMPLES WILL BE COLLECTED BY A TRAINED
TIGARD WATER PERSONNEL WORKER.
22.PROVIDE AN ECLIPSE NO. 88 SAMPLING STATION MANUFACTURED BY
KUPFERTE FOUNDRY, ST. LOUIS, MO 63102. THE SAMPLING STATION SHALL
BE 2-INCH - 0-INCH BURY, WITH A ¾-INCH FIPT (FEMALE IRON PIPE THREAD)
INLET, AND A ¾-INCH UNTHREADED HOSE NOZZLE. ALL SAMPLING
STATIONS SHALL BE ENCLOSED IN A LOCKABLE, NON-REMOVABLE,
ALUMINUM-CAST HOUSING. WHEN OPENED, THE SAMPLING STATION SHALL
REQUIRE NO KEY FOR OPERATION AND THE WATER WILL FLOW IN AN
ALL-BRASS WATERWAY. ALL WORKING PARTS WILL BE OF BRASS AND BE
REMOVABLE FROM ABOVE GROUND WITH NO DIGGING. A COPPER VENT
TUBE (STANDARD) WILL ENABLE THE SAMPLING STATION TO BE PUMPED
FREE OF STANDING WATER TO PREVENT FREEZING AND TO MINIMIZE
BACTERIA GROWTH. THE EXTERIOR PIPING WILL BE BRASS AND A ¼-INCH
BALL VALVE SHALL BE PROVIDED IN PLACE OF THE ¼-INCH PET COCK ON
THE VENT PIPE.
23.WATER METERS WILL BE INSTALLED BY TIGARD WATER PERSONNEL UPON
INDIVIDUAL REQUEST AND PAYMENT BY OTHERS (PROPERTY OWNER). ALL
WATER METERS CONNECTED TO AN IRRIGATION SYSTEM MUST HAVE THE
PROPER OREGON STATE APPROVED BACKFLOW PREVENTION DEVICE
(MINIMUM OF A DCVA) INSTALLED ON THE PROPERTY SIDE OF THE WATER
METER. IN ADDITION, REGARDLESS OF SIZE, EVERY METER FOR
COMMERCIAL, MULTI-FAMILY, INDUSTRIAL, AND INSTITUTIONAL SERVICE
SHALL HAVE THE PROPER OREGON STATE APPROVED BACKFLOW
PREVENTION DEVICE (MINIMUM OF A DCVA) INSTALLED ON THE PROPERTY
SIDE OF THE METER. THE BACKFLOW PREVENTION DEVICE SHALL BE
INSTALLED AND TESTED BY THE PROPERTY OWNER, WITH THE RESULTS
FORWARDED TO THE TIGARD PUBLIC WORKS WATER DIVISION BEFORE
WATER SERVICE CAN BEGIN.
