As requested, Northwest Testing, Inc. (NTI) has provided special inspection for the Durham Primary Sludge Heating
System (Project No. 6368.076). Inspections were provided on an on-call basis. Our special inspection services
included the following activities:

• Inspection of Reinforcing Steel and Placement
• Inspection of Anchors Installed in Hardened Concrete

Department of Land Use & Transportation — Building Services throughout the course of this project. To the best of
NTI's knowledge, based on our inspections, the structural aspects of the project have been constructed in substantial
conformance with the approved drawings, specifications, applicable provisions of the State Building Code, and the
inspection methodology and acceptance criteria established by the project designer(s).

THE FOLLOWING WAS NOTED:
NTI arrived on-site, as requested, to monitor the installation of the heat exchanger grinder pads dowels. No.4 at 48"rebar
dowels were installed in clean holes with 4"minimum embedment using Hilti RE-500-V3 epoxy per 1/M002. To the best of
NTI's knowledge,the subject epoxy anchor installation occurred in general conformance with the project plans, specifications,
ESR-2508,and pertinent codes. NTI informed R.L. Reimers Co. personnel of all observations.

NTI arrived on-site, as requested,for anchor installation inspection of 3/4"x 7.5" SS threaded rod installed at the heat
exchanger. (6)Total anchors were installed in clean holes with 5.5"minimum embedment using Hilti HIT HY-200 A epoxy per
Detail 1/M-002. To the best of NTI's knowledge, the subject epoxy anchor installation occurred in general conformance with
the project plans, specifications, ESR-3187, and pertinent codes. NTI informed R.L. Reimers Co. personnel of all
observations.

THE FOLLOWING WAS NOTED:
NTI arrived on-site,as requested, to provide reinforced concrete placement inspection of Wilsonville Concrete Products
Mix No.40701001A placed as heat exchanger and sludge grinder pads. Fresh properties testing was provided and one set of
five compressive strength cylinders were cast from the load delivered. Test cylinders were stored on-site, overnight, in an
insulated cure box. Slump was exceeded by 3/ inch and batch to discharge time was exceeded by 30 minutes. Batch
weights on load ticket are incomplete and water cement ratio cannot be calculated. To the best of NTI's knowledge, with
noted exceptions,the reinforced concrete placement occurred in general conformance with the project plans,specifications,
and pertinent codes. NTI informed R.L. Reimers personnel of all test results and observations.
