As requested, Northwest Testing, Inc. (NTI) has provided special inspection for the Durham Primary Sludge Heating
System (Project No. 6368.076). Inspections were provided on an on-call basis. Our special inspection services
included the following activities:
Anchors—Adhesive • Material Verification
• Adhesive Anchor Installation Inspection
Concrete Construction • Material Verification
• Inspection of Reinforcing Steel and Placement
• Inspection of Anchors Installed in Hardened Concrete
• Verified Use of Required Mix Design
• Provided Fresh Properties Tests and Cast Compressive Strength
Specimens
• Inspection of Concrete Placement for Proper Application Techniques
• Inspection of Formwork for Shape, Location and Dimensions of the
Concrete Member Being Formed
A complete record of our inspection reports have been forwarded to the project team and the Washington County
Department of Land Use &Transportation — Building Services throughout the course of this project. To the best of
NTI's knowledge, based on our inspections,the structural aspects of the project have been constructed in substantial
conformance with the approved drawings, specifications, applicable provisions of the State Building Code, and the
inspection methodology and acceptance criteria established by the project designer(s).
