FIELD INSPECTION REPORT

04/19/19 - Anchor Load Test
As requested by the client, CTI representative was on site to conduct a load test on anchors that were installed without
inspection.
Engineer's letter dated 03/27/19,written by Robert McClung, indicates a load value of 2,400 lbs to be applied to (2) holdown
anchors installed on back garage wall. Load was applied with a hydraulic ram assembly (CTI N2 4590) and a gauge (CTI
N2 4594)to specified load. The anchors held the required load without cracking,spalling or excessive movement.
The results of CTI's testing were discussed with Nixon Lange prior to leaving site.
