Dear Mr. Van Domelen:
This letter certifies that we have performed special inspection of the referenced project, in accordance with
Chapter 17 of the International Building Code. Special inspection has been performed on (4) push piers, as
shown on the permit documents.
On November 3, 2018, a representative of i.e. Structural observed the installation of (2) Foundation
Supportwork Incorporated model PP288T push piers, as shown on structural drawings. All push piers
reached or exceeded the specified installation pressure and the minimum penetration depth specified on
structural drawings. Load testing was performed on piers(1)and(2)in accordance with ASTM method D1143 a.
(Quick Method), and passed the creep acceptance criteria. i.e. Structural has reviewed the installation logs of
the remaining piers and confirmed them to be acceptable.
Special inspections have been provided to meet the requirements of the building code and do not constitute
a guarantee of the installed work. It is the responsibility of the contractor to execute the work in accordance
with the structural drawings.
