Dear Kristine:
Per your request, we are pleased to provide you this proposal for the re-piping and pump relocation
above the medical records office. Below please find the quote, which includes labor, equipment and
material, as well the following Scope of Service:
Scope of Service:
➢ Draining of system to allow for piping modification. OFFICE COPY
o System will be down for several days.
> Welding of new connections in place of gasketed tees.
➢ Relocation of pumps above space over hallway area.
> Installation of piping from new welded fittings to pumps.
> Pressure testing of new piping to ensure a tight system.
➢ Insulation of new piping.
> Electrical connections for new pumps.
> Includes premium time for off-hours work for shutdown of system.
> Permitting and permit fees.
Note: The work listed above is based on performing during normal business hours. This price is valid for
a period of sixty(60) days.
Regarding payment: Preferred payments made by check. As of March 2018, please note that use of
credit card for payment will include a surcharge of 3% of invoice, we accept Mastercard and Visa.
Note: MacDonald-Miller would provide a full one-year parts and labor warranty on above work.

Clarification Statement:
As a valued customer, we want to inform you that the upcoming decision on Section 232 of the Trade Expansion Act
of 1962 will potentially have a direct impact on the material pricing and product availability on your project. The
Department of Commerce made their recommendation to the President on February 16th 2018, a final decision is
anticipated to be made prior to the April 11th deadline. This decision is anticipated to have a direct impact on the
price of Carbon Steel, Stainless Steel&Aluminum. Currently many of our manufactures and suppliers are
unwilling to quote large orders or long term projects due to this uncertainty. We will keep you informed as
information comes available, but at this time we must inform you that material pricing is subject to change on
products directly tied to the final decision on Section 232 of the Trade Expansion Act of 1962.

ASSUMPTIONS/CLARIFICATIONS/EXCLUSIONS:
❖ Assumes project to be done in the spring for mildest outside conditions as the system will be
down for several days.
❖ Excludes any other work than specifically included in Scope of Service above.
❖ Excludes any overtime.
❖ Excludes any additional scope resulting from or requirements of the Owner, AHJ, or other
subcontractors.
❖ Excludes any structural design and/or modifications to accommodate new equipment.
❖ Excludes any allowance for unforeseen conditions.
❖ Excludes any removal/relocation of existing building components as necessary for new install.
Excludes any hazardous material abatement.
