Shady Lane On May 5 , 1986, a cursory inspection was made of the alteration i process of the public corridor system at the above-noted address. At that time, several deficiencies were noted and require correction i before we can grant our approval for the City of Tigard to issue a Certificate of Occupancy. In the f-rst instance, the gypsum wallboard on the ceilings of all three stories was to have been repaired prior to the installation of the drop ceiling. In some places this was not accomplished, and in +.. ' other places the repair work was somewhat less than adequate. If you will remember, this corridor is fire isolated by an envelope having a one-hour five resistance. In order to accommodate the one-hour fire resistance requirement on the ceiling of the corridur, two (2) layers +' of 5/8" Type X gypsum wallboard is installed on the underside of the a floor ceiling assembly. The repair of penetrations in this gypsum wallboard must therefore include the removal of both layers , and then n 4 subsequently repaired consistent with the manner in which it was originally installed. If you will remove the ceiling panels and look, you will see the less-than-adequate job that was done. In the second instance, all door assemblies subsequently installed in this project must be smoke tight, both within the door and frame as well as the frame and wall . During my inspection, I could see light between the frame and the wall in two of these stairway door assem- blies. Page 2 i In the third instance, we were to have been contacted to make inspec- tions prior to the covering of any wall or partition framing, prior to the installation of the suspended ceiling, and also upon completion of ■ } the construction project. The only call we received and subsequently responded to was in vain as the third floor was the area most com- pleted, and it s >ill had deficiencies noted „,i the inspection notice I left at the project. Please refer to the letter to Mr. David Vonada from Wilburn Dodge, dated February 18, 1986. A copy of this letter has been enclosed for your convenience ,. In the last instance, we would like to witness tests made to the smoke detection system where it is tied into the stairway doors. As a result , we offer a few requirements and comments as follows . In the first instance , the wall notes and symbols" which appear on sheet 10 .3 of the drawings must be applicable to all stories . That is , corridor walls must extr:nd to the gyp board ceiling above . Door assembly 0307 , located at the top of the central stairway , must have a rated and labeled fire resistance of not less than 60 minutes and must have a transmitted temperature end-point rating of not to exceed 450 degrees Fahrenheit above the ambient after 30 minutes of fire exposure . That is , this door assembly now serves an enclosed stairway and must comply with stairway enclo- sure requirements . By way of contrast , door assemblies 315 and 316 , where you call for 60-minute-rated assemblies , need only be 20-minute assemblies and provided with gasketing as prescribed for any common corridor access opening . That is , due to area and height this building is required to be one-hour fire-resistive construction throughout and, as mentioned in our telephone conversation recently, the suspended ceiling system has no fire-resistance rating. While we have no objections to the employment of wired glass and safety features beyond the scope of I fire safety do not fall within our perview, this comment is offered for whatever it may be worth. Exits and the way of access to exits must be identified by exit signs hiving letters not less than E inches in height w=th a principal strokes being not less than 3/4-inch in width. In this regard exit signs must he installed over door assembly nos. 101 and 104. The corridor walls must extend full height to the gypsum i plasterboard substructure attached to the bottom chords of the floor trusses. That is, the exit corridor system must be fully "fire isolated" from the remainder of the building by construction having a one-hour fire-resistance rating. Doorways opening onto the corridor system must be r► fitted with a self-closing and latching smoke and draft control door assembly having a rated and labeled fire re- sistance of not less than twenty minutes. The door assembly, in each instance, must be provided with a seal where the door meets the stop on both sides and across the top. HVAC duct penetrations of the corridor walls must be fitted with fire dampers which are set in collars and which are independent of the duct work. That is, a shear joint must be prGvided where the duct connects to the damper assembly on each side to the effect than the dLCt v,--k may fall away without displacing the fire damper from the wall . 3. All doors, as shown on the drawings, must be openable from the "inside" at all times for immediate exit without the use of a key, special knowledge or effort. 4. Inspection and approval of the project area by this office is required: , r a) prier to cover of the framing elements following the installation of all utility runs and (if appropriate) the firestopping referred to in Item No. 1 . b) upon completion of construction and prior to the occupancy of the project area or any portion of j the project area. 5. One set of approved plans bearing the stamps of the Tigard Building Department and this office must be maintained at the project and must be made available to building and fire inspectors for reference during prescribed construction inspections. 6. Prior to the occupancy of the project area or any portion of the project area, a Certificate of Occupancy or other written authorization must be obtained from the Tigard Building Department.
