we showed the address as Building E-5 and that no Certificate of Occupancy had been issued. Al that
time,(January, 1997), several letters had been sent to Horizon Community (church with no response____
If the corrections have been made since the February 26, 1997 inspection, please call the 24-hour
Inspection_ bine at 639-4175 and request a final inspection. Access to the site will be required from 8-4
the day of the inspection. If it is necessary to meet an Inspector at the site for access,leave that information
on the Inspection Line. We require a "2-hour window of time" for access. For example, you may request
to meet an Inspector on site between 8_10,etc.

JT 02/25/99 2/25/99 numerous attempt;t to "clean up" files associated with
11515 Durham Road. Numerous request3 to hispectors to clarify
inspections, suite numbers, etc . These attempts covered years
1995-1999 . As of this date, unable to get clear determination
from the "field" what tenant was in what space, was work done,
was it inspected? Besides the COT Inspectors, I sent a faxed
request to Bluestone Hockley and had numerous conversations with
Bluestone during 1995-1997 . If anyone wants to research it
further, you should look in all the files attached to this
address, since the suites and tenants are not clear. Also, on
this Bup97-0351 , a C/O was printed, signed by TLP and David
Scott, never mailed out because of discrepancies, then TLP
voided permit stating "tenant moved out" . All the !1515 Durham
Rd files are being sent to records for. microfilming. Nr, f:.;rrher
attempts will be made to clarify these projects with the
Building Inspectors, Inspectors Supervisor, Plans Examiners, or
