The inspection was performed on November 2, 1992, and the following conditions were found: • Sidewalk at the end of bikepath has been damaged, and is in need of replacement of three (3) panels. The city has marked the area for removal on the sidewalk . • Concrete ramp from asphalt bikepath to sidewalk has been completely damaged and is in need of replacement. The City will allow asphalt pavement replacement in lieu of concrete at this transition. The City has marked the area for removal on the ramp. A portion of the asphalt bikepath has been damaged due to construction traffic. The City has marked the area for removal on the asphalt. Asphalt replacement is also needed in Venus Court adjacent to the new driveway approach. Gravel shoulder along Lot #13 has been damaged due to construction activities on the lot . Gravel needs to be replaced along the entire length of lot #13 to prevent undermining and failure of the i bikepath. • An evaluation of the existing Sanitary and Storm Sewer that runs under the existing bikepath is required to ensure that no damage to these systems from construction traffic has occurred. This evaluation will be conducted by the City Field Operations Department, at their- earliest convenience. All repair work shall be -'.n accordance with the City of Tigard standards and details attached to the permit. Pictures of the bike path & 115th Avenue curb/walk shall be taken by the City prior to any use of the path. Permittee is responsible to assure that this has been done before path use occurs . 2 . The City' s standards specification for traffic control is the Manual on Uniform Traffic Control Devices; it shall be adhered to hereinzegard. The City' s construction standards and specifications also apply hereto. 3 . Whenever vehicles are to be on the path, warning signs or flagman will be at both ends of the accessway (path) to assure safe vehicle and pedestrian movement . Advance warning devices and flagmen shall also be provided on S .W. 115th Avenue . 4 . Applicant will be responsible for damage as determined by the City Engineer. 5 . Limit operation from 9 : 00 A.M. to 4 : 00 P .M. Monday through Friday . 6 . Pathway/Roadway will be left in a clean condition after 4 : 00 P .M. every day.
