This letter is a follow up to our telephone conversation on April
16, 1991, in which you informed me that you had r--curned the area
back to its "originally condition" . On April 15, and again on
April 17, 1991, 1 had our inspector visit the site and take some
photos; in addition on April 2.4, 1991 I personally went by the site
to determine what had been done. From my observations it appears
that the area still needs additional. work to return it back to its
"originally condition" .
I would like to meet with you at the site as soon as possible to
came to a mutual resolution to this problem.

APPLICATION FOR SEWER SERVICE
The undersigned agrees, in consideration of the wiwer service connech.:-, by the
Tigard Sanitary District, to abide by and comply with the ordinances, regulations and rules of the Dist-
rict presently in effect or hereinafter enacted and to pay sewer service charges as the same may be im-
posed from time to time when due and 'before such charges become delinquent.
I fiilly understand that all unpaid se,;er service charges become a lien upon the
roperty served as stipulated in O. R. S. 224.20.
Connections to the District's system must be made by b,)nded contractors ano/or
bonded and 1:censed plumbers.
