To Whom It May Concern:
Wednesday, May 21, 2014, I made a special inspection of the installation of the
foundation SET-XP adhesive anchors with the following observations and comments. A chart of
my field measurements is attached.
1. Specified adhesive was verified as SET-XP.
2. All three anchors for Mark 10 were 5/8" dia. instead of 2" dia. as specified. This
change is acceptable.
3. All anchors that were specified to be A193 Gr. B8 (304/31655) were A1554
Gr. 36, galvanized. This change is acceptable.
4. All anchor lengths were longer than specified and final projections were all different
than specified. However, these changes are acceptable.
5. All threaded rod embedment depths were equal or greater than specified, resulting in
equal or greater strengths, which is acceptable.
6. Most concrete edge distances as drilled were different than specified. A thorough
analysis was made of each variance and it was determined that all installed edge
distances did not reduce allowable loads beyond the loads that are required and,
therefore, are acceptable.

In conclusion, all SET-XP anchors passed inspection.
