Inspection Adhesive Anchors

Arrived on site to provide special inspection on the adhesive anchoring of 5/8" diameter threaded
rods for moment frame anchor bolts each s ide of the garage door opening into the concrete stem
wall. Inspected drill bit diameter and embedment depth. Holes were cleaned by compressed air
and wire hole brush. Adhesive used was Simpson SET XP, ICC-ESR-2508,
Lot# 203N260N, E xp: 8/15.
To the best of our knowledge, all items inspected today are in conformance with approved plans
and specifications.
