Date CITY OF TIGARD,OREGON AMENDMENT TO CONTRACT EMERGENCY BRIDGE INSPECTION NORTH DAKOTA STREET AND TIGARD STREET BRIDGES 0150068 AMENDMENT #2 The Agreement between the City of Tigard, a municipal corporation of the State of Oregon, hereinafter called City, and OBEC Consulting Engineers, hereinafter referred to as Consultant, entered into on the 12" day of May,2015,and previously amended on June 2,2015,is hereby amended as follows: 3. Engineer's Fee A. Basic Fee 1) As compensation for Basic Services as described in Exhibit A of this Agreement, and for services required in the fulfillment of Paragraph 1, the Engineer shall be paid on an hourly rate based upon the "Schedule of Rates" in Exhibit B of this agreement, which shall constitute full and complete payment for said services and all expenditures which may be made and expenses incurred, except as otherwise expressly provided in this Agreement. The Basic Fee shall not exceed the amount of Twenty Six Thousand Eight Hundred Two and No/100 Dollars (r46,732.00 $26,802.00) without prior written authorization. EXHIBIT A SCOPE OF SERVICES TASK 4 CONSTRUCTION DESIGN CONSULTATION AND INSPECTION Consultant shall provide consultation and technical services regarding design issues that may arise during construction of the Project. Consultant shall clarify construction contract documents and provide written responses to Requests for Information (RFIs). The design consultation will occur only as required and may be ongoing throughout the Project. Upon request of the City or Contractor.during construction,Consultant shall: • Clarify constructiori contract documents. • Respond to field inquiries. Engage the services of the POR on all matters involving design changes. The Contractor has already been provided with design details and they have been included in this package (see Attachment 1). Consultant shall provide construction project management and field inspections as required during the construction to assist the Contractor and owner in installing the designed repairs and documenting the construction. Consultant shall monitor construction activities during construction of the Project utilizing certified Inspectors and require compliance with the contract documents. Consultant shall provide inspection concurrently with the CC's operation. Assumptions: It is anticipated that the contractor will complete the work in no more than ten (10) working days after mobilizing on-site. Assume five (5) field visits to inspect the contractors work performed by a Field Technician 3. Additional hours have also been budgeted for the PM for coordination and invoices and ten (10) hours for a senior design engineer if consultation is needed during construction. Task 4 Deliverables Consultant shall provide Daily progress reports for each site visit and as-constructed drawings of temporary repairs. TASK 5 DESIGN&REPAIR MODIFICATIONS(CONTINGENCY TASK) Upon written authorization by the City, the consultant shall design modifications during construction to address extra or unforeseen work that is required beyond the original scope of Task 4. Assumptions: This task will be performed on a Time and Materials basis with a Not-to-Exceed amount of$5,000. Prior to performing the work, the consultant will provide the City with a breakdown of design costs based on 2015 OBEC rates. Deliverables Design details for modifications. SCHEDULE AND FEES The Contractor has indicated they are available to mobilize to the site after July 6, 2015 and estimates the work to take no more than 10 working days to complete. All repairs will be complete before July 31, 2015. Consultant shall complete this additional work on a Time and Materials basis with a Not-to-Exceed amount of$10,070.00. The bridge has currently been closed to traffic since June 1, 2015 and will remain closed during the duration of the repairs. OBEC has completed design details for repairs to the bridge, including: timber cap strengthening,timber girder strengthening, and the addition of a pedestrian walk along the south side of the bridge.
