NOTE:
This memo is intended to inform you of our site visit for the epoxy inspection. The
site visit was performed on 9-12-14 at 10:20am with Jesse Tissot. There were a
total of(5) locations where post installed anchors were required, (4)per the
approved plans and(1)as required per the memo "OH Arch 090814". Upon arrival
all of the holes had been predrilled and the appropriate threaded rods and epoxy
were on hand. A final brushing and blowout with compressed air was observed for
each hole and the cleanout was approved. The Epoxy was installed per the
manufacturer's specifications and the treaded rods were inserted. All of the post
installed epoxy anchors noted above were installed to our satisfaction and meet the
structural intent.
