DEQ has determined that CenterCal Properties, LLC, has satisfied the methane mitigation requirements for occupancy of the tenant space at Building J Tenant Space J-3 (Pizzasmith) at the site. The tenant space was previously identified as R-102 in Building R-1. DEQ has confirmed that the appropriate methane mitigation measures have been installed or repaired as part of the remodel for this tenant space. Pre-occupancy methane monitoring of interior areas within Building J Tenant Space J-3 has also been performed and the results indicate that methane gas was not detected in any of the areas. Documentation of the installation and repairs to the methane mitigation measures is provided in the above-referenced report. Pre-occupancy monitoring data for the interior areas within Building J Tenant Space 3-3 (Pizzasmith) is provided in the "Field Reports" prepared by GeoDesign,Inc., for May through June 2014,and located in Appendix A of the report. Methane gas monitoring for these building interior spaces, sub-slab engineering controls, and surrounding monitoring wells will be performed under DEQ oversight in accordance with the DEQ-approved monitoring plan, "Updated Methane Monitoring and Contingency Plan", prepared by GeoDesign, Inc., and dated April 27, 2012. This monitoring plan applies to all the buildings and spaces at the Bridgeport Village Development site. In previous documents, this space has been identified as R-102. The required elements of the construction completion report are summarized in Attachment D (Item 11.1) of the Order of Consent. The Occupancy Approval Letter for tenant space J-3 was issued by DEQ on July 1 7, 2006. For your reference, definitions of all acronyms used herein are defined at the end of this document Tenant improvements in tenant space J-3 began on May 14, 2014 with a pre-construction meeting to accommodate the Pizzasmith restaurant. Improvements to the tenant space included concrete slab cuts and repairs to the HDPE geomembrane to install and/or modify utilities for the new tenant. Methane mitigation work completed during this tenant improvement was completed in general accordance with the May 2, 2014 revised draft Site-Wide Tenant Space Modification Methane Mitigation Plan. GeoDesign previously submitted to DEQ the following reports related to Building J: • Construction Completion Report;Building R 1 and Pad 6;Bridgeport Village Development Site; SW Bridgeport Road; Tigard, Oregon, dated May 16, 2005. • Remodel Construction Completion Report;Building R1 Tenant Space R102(Levi's);Bridgeport Village Development Site;SW Bridgeport Road; Tigard, Oregon, dated June 30, 2006. The sub-slab passive venting system and the HDPE geomembrane were disturbed during the remodel of Building J tenant space J-3. A description of the construction activities, observations, and documentation of repairs to the HDPE geomembrane for Building J tenant space J-3 are summarized in this Remodel Construction Completion Report. The repairs to the HDPE geomembrane in Building J tenant space J-3 were conducted in general accordance with the following documents: • GeoDesign's revised draft Site-Wide Tenant Space Modification Methane Mitigation Plan; Bridgeport Village Development Site;SW Bridgeport Road and SW 72"°Avenue; Tigard and Tualatin, Oregon, dated May 2, 2014. • Dudek&Associates, Inc.'s R1 HDPE Sub-Slab Passive Venting System engineering drawings, revision 0.0, dated November 4, 2004. This Remodel Construction Completion Report does not describe the installation and compaction testing of backfill used in the utility trenches. The installation and testing of trench backfill is not the responsibility of GeoDesign. 2.0 REMODEL CONSTRUCTION COMPLETION REPORT ITEMS The following items are included in the Remodel Construction Completion Report for Building J tenant space J-3: GEODESIGN= 1 BPCenters-2-25:071714 2.1 Observation Personnel 2.2 Contractor 2.3 Work Completed 2.4 HDPE Geomembrane Repairs 2.5 Summaries of Completion 2.6 Final Observations and Results of Verification Screening 2.1 OBSERVATION PERSONNEL The methane mitigation components repaired at Building J tenant spaceJ-3 were observed by GeoDesign personnel. GeoDesign observed the exposed portions of the sub-slab sand conveyance layer and HDPE geomembrane while the repairs were being completed. The qualifications of GeoDesign's field representative are described in GeoDesign's CQA Plan in Appendix D of GeoDesign's Final Design Report;Revision 2.0;Bridgeport Village Development; Former Durham Quarry;Tualatin and Tigard, Oregon, dated September 30, 2004 and the revised draft Site-Wide Tenant Space Modification Methane Mitigation Plan;Bridgeport Village Development Site;SW Bridgeport Road and SW 72'Avenue; Tigard and Tualatin, Oregon, dated May 2, 2014. 2.2 CONTRACTOR The HDPE geomembrane was repaired by ACF West Construction (ACF West) of Portland, Oregon. 2.3 WORK COMPLETED GeoDesign was notified by Pacific Crest Construction that concrete and HDPE geomembrane would be cut on May 19 and 20, 2014 and repairs would begin on June 2, 2014 at Building J tenant space J-3. Approximately 360 square feet of concrete slab were cut and removed from tenant space J-3 for the modification of sanitary sewer and waste utilities and for the placement of electrical utilities. GeoDesign conducted periodic methane monitoring during construction observation activities. Methane was not detected in the concrete slab cut areas or in the tenant space after the HDPE geomembrane was removed. Repairs to the HDPE geomembrane were performed by ACF West personnel and observed and documented by GeoDesign. The HDPE geomembrane was repaired in general accordance with the plans and specifications contained in the documents referenced in Section 1.0. Field reports, field drawings, project site photographs, daily reports, trial weld logs, and summaries of completion are provided in Appendices A through F, respectively. Repairs to the HDPE geomembrane and sub-slab passive venting system were completed by ACF West from June 2 through 4, 2014 and June 24, 2014. On June 2, 2014, ACF West prepared and tacked approximately 300 square feet of HDPE geomembrane patch with a Leister Triac welding tool and installed seven pipe boots. On June 3, 2014, ACF West continued the HDPE geomembrane repairs with the installation of three pipe boots, 250 linear feet of extrusion weld, 20 square feet of HDPE geomembrane patch, and 4 linear feet of flat corrugated vent pipe. On June 4, 2014, ACF West completed the HDPE geomembrane repairs with the installation of five pipe boots, 140 linear feet of extrusion weld, 10 square feet of HDPE geomembrane patch, 37 linear feet of aluminum batten strip, 74 linear feet of double-sided tape for the batten strip, six tubes of sikaflex for the batten strip and pipe penetrations, and the completion of non- GEODESIGN? 2 BPCenters-2-25:071714 destructive integrity tests. On June 24, 2014, ACF West completed a repair on an accidental membrane puncture by spot welding the puncture. A detailed description of the sub-slab sand conveyance layer, HDPE geomembrane repairs, and QC methods are described in Section 2.4. The field drawings showing the locations of the slab cuts and repairs at tenant space J-3 are included in Appendix B. A summary of the repairs and associated quantities are as follows: • Approximately 330 square feet of HDPE geomembrane were repaired. • Approximately 390 linear feet of extrusion welds were completed. • Fifteen pipe boots were installed for utility penetrations through the HDPE geomembrane and concrete slab. • Approximately 37 linear feet of aluminum batten strip and 74 linear feet of double-sided tape were installed onto footings and concrete collars. • Six tubes of sikaflex were used to seal the batten strips and pipe penetrations. • Four linear feet of flat corrugated vent pipe were installed. • An accidental membrane puncture was repaired by spot welding approximately 3 linear inches of HDPE extrudate over the puncture. 2.4 HDPE GEOMEMBRANE REPAIRS HDPE geomembrane repairs were completed at tenant space J-3 on June 4, 2014, with the exception of an accidental membrane penetration and subsequent repair occurring on June 24, 2014. Periodic methane monitoring was conducted during construction observation activities using a calibrated Landtec GEM-2000 methane meter at multiple sampling points until the HDPE geomembrane repairs were complete. Methane was not detected at the monitoring locations within the tenant space. Approximately 6 to 12 inches of HDPE geomembrane were exposed along the perimeter of the concrete slab cut. Prior to installing the HDPE geomembrane, the 2-inch-thick conveyance layer of sand was replaced and observed to be smooth, absent of stones or dirt clods greater than 1/4 inch, and free of standing water. Based on GeoDesign's observations, it is our professional opinion the sand conveyance layer in the repaired areas of the tenant space was installed in general accordance with the specifications. A patch consisting of 60-mil-thick HDPE geomembrane was installed over the cut area. The HDPE geomembrane used for the repair was from HDPE geomembrane Roll No. 103179768. Laboratory conformance data for the HDPE roll was provided by GSE Lining Technology, Inc. of Houston, Texas. The samples conformed to the material specifications. The conformance samples were analyzed for average and minimum thickness, density, carbon content, carbon black dispersion, tensile strength, elongation, tear resistance and puncture resistance by ASTM Methods D 5199, D 1505, D 4218, D 5596, D 6693, D 1004, and D 4833, respectively. The HDPE geomembrane patches were cut to extend a minimum of approximately six inches past the edges of the existing HDPE geomembrane and were tack-welded in place using a Leister- Triac welding tool. The edges of the boot/patch and geomembrane to be welded were then lightly abraded using a grinding wheel. An extrusion welder was used to weld the HDPE Wir DESIGN? 3 BPCenters-2-25:071 714 geomembrane seams and complete the repair. The entire length of each weld was tested using a vacuum box, with the exception of approximately 4 inches of weld inaccessible to the vacuum box equipment due to space limitations and utility conflicts. Based on our observations, the weld completely covered the seam and was in good condition. Therefore, it is our professional opinion that the welded seams will adequately mitigate methane intrusion into the tenant space. Trial extrusion welds and destructive strength tests were completed at the start of welding activities, at mid-shift, and at the end of welding activities. Based on the destructive strength test results, the trial weld seam strengths met or exceeded the minimum peel adhesion strength of 78 ppi and the minimum shear strength of 121 ppi as required by the specifications. A record of machine temperature settings, peel adhesion strengths, and shear strengths are provided on the trial weld logs in Appendix E. According to the specifications, destructive seam field testing is required at a frequency of one destructive test per 1,500 linear feet of seam length. The total seam length extrusion welded for all repairs was approximately 390 linear feet, which is less than the specified requirements. Therefore, destructive tests were not necessary and were not conducted. As part of the repairs, 15 pipe boots were placed over the pipe penetrations in general accordance with the specifications. Approximately 37 linear feet of batten strip, including 74 linear feet of double-sided tape and anchors placed at 6-inch centers were installed. Six tubes of sikaflex were used to seal the batten strips, pipe penetrations around concrete-collars, and over two foam-filled outer pipes containing p-trap primer lines. Lastly, the protective sand layer was restored prior to pouring the concrete slab over the repaired areas. On June 24, 2014, GeoDesign was contacted by Pacific Crest Construction regarding an accidental membrane puncture. GeoDesign personnel observed ACF West repair the membrane by placing approximately 3 linear inches of extrusion weld over the puncture area. The repair was not accessible to the vacuum box for non-destructive seam testing. ACF West conducted trial weld testing before and after welding activities, and GeoDesign personnel closely observed the repair weld. Based on our observations, the weld completely covered the accidental membrane puncture and was in good condition. Therefore, it is our professional opinion that the repair weld will adequately mitigate methane intrusion into the tenant space. The protective sand layer was restored prior to pouring the concrete slab over the repaired area. 2.5 SUMMARIES OF COMPLETION GeoDesign and the installer representative (Mr.Jeff Boys of ACF West) signed summary of completion reports for the remodel of Building J tenant space J-3, stating the following: • A total of 390 linear feet of extrusion welding was completed on 330 square feet of HDPE geomembrane. The trial welds passed the specification requirements. The HDPE repair welds passed the vacuum box test on accessible seams. • Fifteen pipe boots were placed around utility penetrations through the HDPE geomembrane. • A total of 37 linear feet of aluminum batten strips and 74 linear feet of tape for batten strips were installed. • Six tubes of sikaflex were used for the batten strips and pipe penetrations. G EODESIGN_ 4 BPCenters-2-25:071714 • Four linear feet of flat corrugated vent pipe were installed. • An additional mobilization was completed for incidental membrane repairs with a total of 3 additional inches of extrusion weld and two trial welds for weld testing. Summaries of Completion for the tenant space are included in Appendix F. After the HDPE geomembrane repairs were completed, the general contractor restored the protective sand layer and concrete slab to match pre-existing conditions. Digital photographs of the protective sand layer are included in Appendix C. 2.6 FINAL OBSERVATIONS AND RESULTS OF VERIFICATION SCREENING The final repair observations and pre-occupancy screening of Building J tenant space J-3 were conducted by GeoDesign personnel on June 19, 2014. GeoDesign observed and documented the repair of the methane mitigation systems (gas conveyance sand layer, sub-slab passive vent piping, and HDPE geomembrane) impacted by the remodeling activities. Methane verification screening was conducted at seven locations within tenant space J-3 using a calibrated Landtec GEM-2000 methane meter. The locations were selected on the potential for methane accumulation. Methane was not detected at the selected locations within the tenant space during the verification sampling activities. Based on our observations and methane verification sampling, it is our professional opinion that the gas conveyance layer, sub-slab passive vent piping, and HDPE geomembrane have been repaired and successfully tested in accordance with the applicable specifications outlined in the revised "draft"Site-Wide Tenant Space Modification Methane Mitigation Plan, dated May 2, 2014. Copies of the Summary of Completion reports for the repairs at the remodel in tenant space J-3 are included in Appendix F. GeoDesign recommends that the DEQ approve occupancy of Building J tenant space J-3 (Pizzasmith). Routine methane monitoring is currently completed on a semi-annual basis.
