AMENDMENT TO CONTRACT
AUDIOMETRIC SERVICES AND TRAINING
AMENDMENT #2011-01
The Agreement between the City of Tigard, a municipal corporation of the State of Oregon, hereinafter
called City, and Industrial Hearing Service, Inc. (IHS) , hereinafter referred to as Contractor, entered into on
the 15th day of July,2009,is hereby amended as follows:
• Compensation will be amended as follows:
City agrees to pay Contractor an amount not to exceed two thousand wee six hundred dollars
($ 300 2,600) annually for performance of those services described in this Agreement. Payment
will be made based on Contractor's invoice, subject to the approval of Loreen R. Mills,Assistant
to the City Manager, and not more frequently than monthly. Payment shall be payable within
thirty (30) days from the date of receipt by the City.
• Exhibit"A" Scope of Work will be amended by inserting the following language:
"The following item will be accomplished at a cost of$5.00 for each test and in compliance with
all services identified above except for conducting the actual audiometric test:"
16. "Employee audiometric records received from clinics other than IHS will be processed as if
they were conducted by IHS."
IN WITNESS WHEREOF, City has caused this Amendment to be executed by its duly authorized
undersigned officer and Contractor has executed this Amendment upon signature and date listed below.
