Under the provisions of the Uniform Fire Code with Tualatin Valley Fire and Rescue
amendments, a provision of their amendment requires the applicant submit with the building
permit application the attached Fire Department Building Survey and Plans.
Instructions for completing the form can be found at the Tualatin Valley Fire and Rescue
Web site at www.tvfr.com. At the top of the page, click on Departments and select New
Construction under Fire Prevention. The Building Survey Form Instructions are listed
under Building Survey.

FIRE DEPARTMENT BUILDING SURVEY AND PLANS
