Professional Service Industries, Inc. (PSI) is writing this letter to document that, in accordance with Section
1704.1.2 of the State Building Code representatives from our firm performed special inspection during construction
for the following project:
Permit No: BUP2013 -0184
Project Address: 10330 SW Greenburg Road, Tigard, Oregon
Project Description: Red Lobster #457
Our project files indicate that PSI special inspection activities listed below were conducted as scheduled from
August 27, 2013 through October 17, 2013. This letter represents a summary of work observed and reported by PSI
representatives and the resolution of any documented noncompliance.
• Compaction of Fill Material
• Structural Steel: Shop Fabrication and Field Welding
♦ Adhesive Anchors
• Reinforced Concrete
To the best of our knowledge, the periodic special inspections referenced herein were performed by our firm in general
accordance with the requirements, approved plans and specifications, provided change orders that impacted plans
and/or specifications, and applicable workmanship provisions of the State Building Code and Standards.
