Houser, CEG WEATHER: Cloudy, -50 degrees F PURPOSE OF VISIT: Subgrade Observation Carlson Geotechnical (CGT) staff, Ryan T. Houser, CEG, arrived on site at 10:40 a .m. at the request of Matt Hubei (excavation contractor). The purpose of my visit was to observe the exposed subgrade on a portion of the site. I met with Matt at the site. The area exposed today was located approximately as shown on the attached Site Plan. The area measured approximately 80 feet (east -west) by 33 feet (north- south). The exposed subgrade consisted of native sandy silt (ML). Portions of the excavation had been overexcavated by approximately 1 foot due to the presence of roots. The subgrade observed today ranged from approximately 1 to 2 feet below the previously existing grade. I probed the exposed subgrade soils with a '/2- inch - diameter foundation rod, and was generally able to penetrate 8 to 10 inches using moderate to strong pressure, indicating soft soils. CGT encountered the soft soils during our investigation, and recommended that the design incorporate a minimum of 16 inches of imported granular structural fill under all footings, floor slabs, and pavements. We understand from Tualatin Valley Fire and Rescue that our recommendations were incorporated into the final plans. The plans were not on -site during our observation today. The approved construction plans should be available on -site for the remainder of construction at the site. We are relying on the contractor to ensure that subgrade elevations are appropriate to provide a minimum of 16 inches of imported granular fill under all footings, floor slabs, and pavements. The subgrade observed today was prepared in general accordance with our recommendations. Matt indicated that they would cover the subgrade with geofabric and place granular fill over the fabric. The imported granular fill will come from two sources: Tigard Sand & Gravel, and Baker -- Cobb Rock, and will consist of 1'/rinch -minus crushed rock. The rock was not available on -site at the time of our site visit, but we understand that it will be delivered from each source in the early afternoon. I left the site at 11:15 a.m. CGT representative Joseph Frank returned to the site at 1:30 p.m. to collect samples of the rock from each of the sources. Joseph returned the samples to the CTI laboratory to have the moisture-density relationship determined for each. David Irish WEATHER: Showers, -45 degrees F PURPOSE OF VISIT: Subgrade Observation Carlson Geotechnical (CGT) staff, David Irish, CEG, arrived on site at about 8:50 a.m. at the request of Matt with DBEC Excavation & Construction (DBEC). The purpose of my visit was to observe the exposed subgrade soils on a portion of the site. I met with Dave Roberts, with DBEC, at the site. Observations (4/21/2008) At the time of my arrival, I observed that the DBEC crew was using two CASE CX130 track hoes to strip the site of vegetation. Dave Roberts requested I visit the site several times during the day to observe document the crew's progress. A total of four visits to the site were made today. The following summarizes my observations today: • The locations of the exposed subgrade soils observed today are shown on the attached site plan. • In general, excavation depths of about twelve inches below the existing grass covered surface were necessary in order to expose the native, sandy silt (ML). • Any roots larger than 1 4-inch in diameter were removed from the subgrade soils. • Probing the exposed subgrade soils with a Winch- diameter soil probe and using moderate effort, I was generally able to penetrate the exposed subgrade soils 8 to 10 inches, indicating soft soil conditions. • The crew from DBEC covered the exposed subgrade soils with geofabric and then 1 - inch -minus crushed rock, imported from Cobb Rock Products, was placed over the geofabric. The purpose of my visit was to observe the exposed subgrade soils on a portion of the site. I met with Dave Roberts, with DBEC, at the site. Observations (4/22/2008) At the time of my arrival, 1 observed that the DBEC crew was using a CASE CX130 track hoe to strip the site of vegetation. I was informed that due to the wet weather no more site stripping would be completed today and the DBEC crew would be concentrating on placing the first two feet of fill (1%-inch-minus crushed rock from Cobb Rock Products). The sample of 1%-inch-minus rock from Cobb that was taken to the CTI lab revealed that the sample contained approximately 48% oversize material. A second sample of the crushed rock was taken to the lab today. It was determined that the second sample contained approximately 36% oversize material. Due to the percentages of oversize material within the samples (both over 30 %) it was determined that testing the density of the fill with a nuclear density gauge would be unreliable and compaction of the imported granular fill would be confirmed by proof - roll observation. The purpose of my visit was to observe the exposed subgrade soils on a portion of the site. I met with Matt at the site. Observations (4/23/2008) At the time of my arrival, 1 observed that some of the DBEC crew was using a CASE CX130 track hoe to strip the site of vegetation, and some of the DBEC crew was using a CASE 750K dozer to spread granular fill material, (1 crushed rock from Cobb Rock Products), over the previously stripped building and parking area subgrade. Several trucks were delivering the fill material which, once spread out, was compacted by several passes of a Dynapac CA152 single, smooth -drum, vibratory roller. The final fill totals for the site range from 9 inches (northern edge) to approximately 6 feet (southern edge). So that no more than 2 vertical feet of fill was placed between proof -roll tests, Matt requested that I visit the site multiple times during the day. The following summarizes my observations today: • A total of three site visits were made today. • The locations of the exposed subgrade soils observed today, the area proof- rolled observed today, and the elevations of the fill at the end of the day are shown on the attached site plan. • Elevations were provided by DBEC and are approximate. • Matt informed me that the finished floor grade was at +236 feet. • In general, excavation depths of about twelve inches below the existing grass covered surface were necessary in order to expose the native, sandy silt (ML). • Probing the exposed subgrade soils with a 'A- inch - diameter soil probe and using moderate effort, I was generally able to penetrate the exposed subgrade soils 8 to 10 inches, indicating soft soil conditions. • To remove soft soils and tree roots, an approximate 15 -foot by 15 -foot area located within the central area of the western side of the parking lot was over excavated approximately 6 inches. • Undocumented fill (brown silt and pieces of asphalt) was uncovered within an approximate 5 foot by 15 -foot area located at the south end of the southeast entrance driveway. Because of safety concerns the fill was left in place, but over excavations of up to 2 feet will be necessary to remove the undocumented fill. • • Proof -roll tests were performed with a fully loaded, dual -axle dump truck. • Proof -roll tests revealed spotty, minor deflection of the fill material. David Irish WEATHER: Overcast, -43 °F PURPOSE OF VISIT: Proof -Roll Observation Carlson Geotechnical (CGT) staff, David Irish, arrived on site at about 6:45 a.m. at the request of Matt with DBEC Excavation & Construction (DBEC). The purpose of my visit was to observe a proof -roll of the granular fill on a portion of the site. I met with Matt at the site. Today I observed a proof -roll of the granular fill (1W -minus crushed rock imported from Cobb Rock Products) that was placed within the building pad area for the new fire station to be built on -site. The following summarizes my observations today: • The locations of the area proof - rolled observed today, and the elevations of the fill at the time of the proof -roll are, shown on the attached site plan. • Elevations were provided by DBEC and are approximate. • Matt informed me that the finished floor grade was at +236 feet. • Proof -roll tests were performed with a fully loaded, dual -axle dump truck. • Proof -roll tests revealed spotty, minor deflection of the fill material. These areas were located along areas of the fill that appeared to have fewer fines, or appeared 'honey'. I indicated to Matt that heavy construction equipment and /or wet weather may have an adverse effect on the fill and /or subgrade. Based on my observations the area proof - rolled today has been prepared in general accordance with CGT's recommendations. The purpose of my visit was to observe the exposed subgrade soils on a portion of the site. I met with Matt at the site. Observations (412312008) At the time of my arrival, I observed that some of the DBEC crew was using a CASE CX130 track hoe to strip the site of vegetation, and some of the DBEC crew was using a CASE 750K dozer to spread granular fill material, (1%-inch-minus crushed rock from Cobb Rock Products), over the previously stripped building and parking area subgrade. Several trucks were delivering the fill material which, once spread out, was compacted by several passes of a Dynapac CA152 single, smooth -drum, vibratory roller. The final fill totals for the site range from 9 inches (northern edge) to approximately 6 feet (southern edge). The following summarizes my observations today: • The locations of the exposed subgrade soils observed today, the area proof - rolled observed today, and the elevations of the fill at the end of the day are shown on the attached site plan. • Elevations were provided by DBEC and are approximate. • Matt informed me that the finished floor grade was at +236 feet. • In general, excavation depths of about twelve inches below the existing grass covered surface were necessary in order to expose the native, sandy silt (ML). • Probing the exposed subgrade soils with a 1/2-inch-diameter soil probe and using moderate effort, I was generally able to penetrate the exposed subgrade soils 8 to 10 inches, indicating soft soil conditions. • Proof -roll tests were performed with a fully loaded, dual -axle dump truck. No significant deflection was observed during the proof -roll today. The area proof- rolled included the majority of the building pad, which is at approximately 12 inches below finished floor elevation. See attached figure for proof -roll area. • Parking lot areas on the west and north sides of the building pad were not at finished grade yet. Work on these areas will continue next week. • I observed that the excavations for the two eastern entrances onto Walnut Street will likely encounter up to 2 feet of undocumented fill materials associated with Walnut Street grading. 1 discussed with Matt that all concrete structural elements, including footings, floor slabs, and parking lot surfaces should be founded on a minimum of 16 inches of imported granular 04/28/08 M0N 15:28 FAX 503 601 8254 Carlson Geotechnical al004 TVFR Fire Station 50 Walnut' Tigard, Oregon CGT Project No. G0602953.A April 25. 2008 material. The purpose of my visit was to observe the exposed subgrade soils on a portion of the site. I met with Rick, with DBEC, at the site. Proof-roll Observation: Today I observed a proof -roll of the parking area at the north end of the proposed fire station. The approximate area proof - rolled today is marked out on the attached site plan (elevations were provided by DBEC). Rick informed me that there was approximately 2.5 feet of fill (1' /z"- minus crushed rock from Cobb Rock Products) to be placed within the area proof - rolled to reach - finished elevations. The proof -roll test was performed using a fully loaded, dual axle, dump truck and revealed areas of minor pumping and rutting as the tires of the dump truck passed over the fill. I informed Rick of my observations and that I observed the fill to be below optimum moisture for compaction. I recommended that the fill be watered down and recompacted. I informed Rick that no more fill should be placed within the area indicated until another proof -roll test could be observed by CGT personnel. Rick indicated that he understood and agreed with my recommendations. Based on my observations the area proof - rolled today has not been prepared in general conditions with CGT recommendations and additional proof -roll testing will be necessary. Subgrade Observation: I observed the DBEC crew excavate the southwest corner for the proposed fire station building. The area observed measured approximately 28 feet (east -west) by 38 feet (north- south). Most of the area was over - excavated 4- inches to allow for a minimum 16 -inch rock section. The only exception was the northwest corner of the area, which was over - excavated approximately 2 feet to remove large tree roots. The area that was over - excavated approximately 2 feet measured approximately 10 feet by 10 feet. The exposed subgrade soils consisted mainly of native, brown, moist, silt. Probing the exposed subgrade soils with a 'inch- diameter soil probe and using moderate effort, I was generally able to penetrate the exposed subgrade soils 8 to 10 inches, indicating soft soil conditions. While on -site, Rick brought to my attention that the basement of the previously existing residential structure located on the site had been backfilled prior to the DBEC crew's arrival on- site. I observed that the basement excavation was located within the southwest corner of the proposed parking area and measured approximately 20 feet (east -west) by 40 feet (north - south). I recommended to Rick that the undocumented fill be removed from the excavation and that the excavation be backfilled with structural fill. Rick indicated that he understood and agreed with my recommendations. Based on my observations the subgrade soils observed within the southwest corner of the proposed building have been prepared in general conditions with CGT recommendations. Basement Overexcavation DBEC requested that I observe a proof -roll of the basement backfill of the previously existing residential structure located on the southwest portion of the site. The proof -roll was conducted using a fully loaded, dual axle, dump truck. I observed significant deflection of the fill soils during the proof -roll. I discussed these observations with Rick and Dave of DBEC, who informed me that they would over - excavate the fill soils to firm native subgrade. Rick requested that I remain onsite during the removal operations. I observed as DBEC removed approximately 2 feet of 1- to 11/2- inch -minus crushed rock. The rock appeared suitable for reuse as structural fill, so it was stockpiled to one side of the excavation. The next approximately 1'/2 to 2 feet of material removed from the excavation consisted of debris -laden soil. The debris consisted of concrete fragments to 18 inches in length, wood, glass, brick, and insulation. This material was stockpiled for later disposal off -site. The finished excavation measured approximately 28 feet (east -west) by 31 feet (north- south). The excavation was approximately 51/2 feet below finished parking lot elevation (according to DBEC) at the south end, and approximately 4'% feet below finished parking lot elevation at the north end. One area measuring approximately 5 feet by 5 feet was overexcavated to a depth of approximately 2 feet lower than the remainder of the basement, due to the presence of additional debris. These areas are indicated on the attached figure. The purpose of my visit was to observe the imported granular structural fill, which had been placed within the footing overexcavations. I met with Rick at the site. The subgrade of the southern footings had been overexcavated previously to allow for a minimum of 16 inches of imported granular structural fill to be placed beneath the footing subgrade elevation (see field report dated May 1, 2008). DBEC had placed 16 inches of imported granular structural fill within the excavations, and had compacted it with a roller. The granular fill had been wetted by hose from a water truck, and appeared to be within an acceptable range of moisture content. I observed additional compactive effort on the granular fill within all of the footing trenches for the fire station using a roller. I did not observe any additional compression /deflection of the fill, indicating a well -keyed condition. Based on my observations and probing the footing subgrade has been prepared in general conditions with CGT recommendations. In addition, I observed the exposed subgrade soils within a portion of the parking lot, as shown on the attached figure. The exposed subgrade soils consisted of native, brown, moist, silt (ML). Generally, I was able to penetrate the exposed soils 8 to 10 inches using a %"- diameter soil probe and moderate effort, indicating soft soil conditions. Based on my observations and probing the parking lot subgrade observed today has been prepared in general conditions with COT recommendations. At the time of my arrival, the contractor had begun placing fill (reused 1 /cinch -minus crushed rock) within the basement excavation. The basement excavation measured about 30 feet (east - west) by 32 feet (north- south) by 5- to 5 -feet below finished parking elevation, and was located at the southeast corner of the site. While on -site, I observed as the contractor placed and compacted about 21/2 feet of fill within the basement excavation. The fill was placed and compacted in two lifts. The first lift was about 18 inches thick and the second 12 inches thick. Utilizing a single drum roller (Dynapac CA 152), the contractor compacted each lift of the fill until no significant deflection was noticed under the applied load of the roller, indicating that the fill had been adequately compacted. According to Rick, the final elevation of backfill was approximately 1 to 2 -feet below planed subgrade elevation. At about 2:55 p.m. Rick indicated that DBEC was shutting down for the day and requested that CGT returned the next day, April 30, 2008 at 8:30 to continue subgrade observations. Base on my observations, the basement backfill was prepared in general accordance with COT recommendations. The purpose of my visit was to observe the exposed subgrade soils within the central approach. I met with Rick, with DBEC, at the site. I observed that there was a utility vault and two above ground utility boxes located along the southern edge of the central approach. Rick indicated that the utility vault and boxes would be removed at a later date. I informed Rick that CGT would need to observe the subgrade, and any resulting backfill placement, after the removal of the utilities. It was decided that the central approach subgrade would be revisited after the removal of the utility vault and boxes. Houser, arrived on site at about 8:00 a.m. at the request of Rick with DBEC Excavation & Construction (DBEC). The purpose of my visit was to observe the exposed subgrade soils on a portion of the site. I met with Rick, with DBEC, at the site. Southeastern Approach: Upon my arrival, Rick informed me that the southeastern approach (see attached figure) had been excavated to subgrade elevation (approximately 1 foot below previously existing grade). The subgrade consisted of a mixture of fill types, including granular fills, asphalt and concrete debris to 18 inches in diameter (rubble), and very soft, wet, clayey silt. The undocumented fill soils were likely associated with utility trenches. I observed the DBEC crew excavate through a portion of the very soft silt and rubble to native subgrade and firm construction sand (surrounding the utilities). The utility lines were not exposed within the overexcavation. Rick informed me that they would over - excavate the area as discussed, and requested that we return to the site at 11:00 a.m. to observe the prepared subgrade within the southeastern approach. I left the site at approximately 9:00 a.m. I returned to the site at approximately 11:10 a.m. DBEC had over - excavated the southeastern approach to firm native subgrade soils. Using a' /r inch - diameter foundation probe and moderate effort, I was generally unable to penetrate the subgrade soils more than 6 inches, indicating soft to medium stiff consistency. The subgrade area observed was the total width (35 feet east -west) of the southeastern approach, which extended from approximately the curb line to approximately 25 feet north of the curb line, where it tied into the previously observed subgrade. The overexcavated portion extended from approximately 4 to 20 feet north of the curb line, and was the total width of the southeastern approach. The depth of overexcavation within this area was from 12 to 24 inches below the proposed subgrade, averaging approximately 18 inches of overexcavation. Central Approach: I observed as DBEC excavated the eastern portion of the central approach to the proposed subgrade elevations. The exposed subgrade soils consisted mainly of native, brown, moist, silt, which probed soft to medium stiff. Portions of the central approach were underlain by up to approximately 12 inches of undocumented fill. David Irish WEATHER: Showers, —50°F PURPOSE OF VISIT: Footing Subgrade Observation Carlson Geotechnical (COT) staff, David Irish, arrived on site at about 11:40 a.m. at the request of Matt with DBEC Excavation & Construction (DBEC). The purpose of my visit was to observe the exposed subgrade soils within the footing excavations for the trash receptacle retaining wall located within the northwest portion of the back parking area. I met with Matt at the site. Today I observed the exposed subgrade soils of the north, east -west trending footing of the trash receptacle retaining wall. The footing had been excavated to a depth of 18 inches below bottom of footing grade. The exposed subgrade soils consisted of native, brown, moist silt, (ML). I was able to penetrate the exposed footing subgrade soils 6 to 8 inches using a Y2-inch diameter soil probe and moderate effort, indicating soft soil conditions. The subgrade within the footing excavation will be covered with geotextile fabric and badkfilled with 1Y2 -inch crushed rock imported from Cobb Rock Products. The remainder of the trash receptacle retaining wall footings will be benched into the already existing parking area fill which also consists of 1t" -inch crushed rock imported from Cobb Rock Products. CGT will need to observe all the footings once they are excavated and the crushed rock is compacted. Based on my observations and probing, the trash receptacle retaining wall footing observed today was prepared in general accordance with CGT recommendations. The purpose of my visit was to observe that the granular backfill within the northern footing excavation for the trash receptacle retaining wall located within the northwest portion of the back parking area had been compacted. I met with Matt at the site. The subgrade soil within the footing excavation was covered with geotextile fabric and backfilled with approximately 16 inches of 1Y2 -inch -minus crushed rock imported from Cobb Rock Products. A jumping jack had been used to compact the fill, which appeared to be well - keyed. The remainder of the trash receptacle retaining wall footings will be benched into the already existing parking area fill which also consists of 1 -inch crushed rock imported from Cobb Rock Products. Based on my observations and probing, the trash receptacle retaining wall footings observed today were prepared in general accordance with CGT recommendations. All footings will be bear on a minimum of 16 inches of imported granular structural fill. David Irish WEATHER: Overcast, —55 °F PURPOSE OF VISIT: Footing Subgrade Observation Carlson Geotechnical (CGT) staff, David Irish, arrived on site at about 9:00 a.m. at the request of Matt with DBEC Excavation & Construction (DBEC). The purpose of my visit was to observe pad footings located southeast of the main structure. No one from DBEC was un site during this site visit. Today I observed that the excavations for the four pad footings located within the southeast quadrant of the site had been backfilled withiYrinch crushed rock imported from Cobb Rock Products. The pad footings are located at the following approximate grid locations; 3.5 - F.5, 3.5 - E.8, 3.5 - D.7, and 3.5 - D.3. CGT did not observe the excavations prior to the placement of the crushed rock backfill and therefore cannot comment on the underlying subgrade or the depth of the excavations. With a soil probe and little effort I was able to penetrate the crushed rock within the footing excavations at least 3 inches, indicating loose soils. The crushed rock appeared below optimum moisture. A CGT representative should be on site when the crushed rock is removed to verify the depth of the excavation and the underlying subgrade soil conditions. I probed the subgrade within the pad footings located at the following approximate grid locations; 3.5 -F.5, 3.5 -E.8, 3.5 -0.7, and 3.5 -D.3, with a Winch - diameter foundation rod. Using moderate pressure, I was generally able to penetrate the subgrade soils 4 to 5 inches, indicating soft soils, consistent with those found during our geotechnical investigation of the site. I observed as the crew from DBEC backfilled the excavations with 11/2-inch-minus crushed rock imported from Cobb Rock Products to bring them to footing subgrade elevation. They compacted the backfill with a jumping jack until well- keyed. DBEC placed a minimum of 16 inches of crushed rock under the footings. Based on our observations today, the pad footing subgrade and structural fill at the referenced grid locations were prepared in general accordance with our geotechnical recommendations. The purpose of my visit was to observe the subgrade of the southern thickened edge slab. I met with Juan of DBEC at the site. The southern thickened edge slab had been overexcavated to a depth of 16 inches below the bottom of the thickened edge slab in the area indicated on the attached plan. The exposed subgrade consisted of native, brown, sandy silt (ML). I probed the subgrade within the overexcavation, with a %- inch - diameter foundation rod. Using moderate pressure, I was generally able to penetrate the subgrade soils 4 inches, indicating soft soils, consistent with those found during our geotechnical investigation of the site. The crew from DBEC stated that they would place fabric within the excavation and backfill the excavations with re -used 1%-inch-minus crushed rock (Cobb Rock) previously excavated from the footing area to bring the grade to footing subgrade elevation. They requested that I return at 12:00 p.m. today to continue my observations. I left the site at 9:15 a.m. I returned to the site at 12:00 p.m. to continue my observations. I observed the crew from DBEC as they used a jumping jack to perform additional compaction on the rock placed within the excavation. I observed no additional deflection during the additional compactive effort, indicating a well keyed condition. DBEC placed a minimum of 16 inches of crushed rock under the footing. In addition, I observed the northern thickened slab edge excavation. The excavation was approximately 12 inches below the surrounding gravel elevation, and had approximately 3 to 5 feet of imported gravel underlying the footing subgrade elevation. CGT previously observed the placement and compaction of the imported gravel within this area. I also observed the footing excavation for the generator enclosure located near the northwest corner of the building (see attached figure). The footing subgrade consisted of previously compacted and observed imported gravel. The footing will be supported by approximately 5 to 6 feet of gravel. Based on our observations today, the southern and northern thickened slab edge subgrades, related structural fill, and the generator enclosure footing subgrade, were prepared in general accordance with our geotechnical recommendations.
