(CTI), provided geotechnical consulting services during repair of a sewer line trench that had experienced on -going settlement within SW 125 Avenue. In addition, our consulting services included observations and testing during construction of the architectural wall near the south margin of the site. These services were conducted on an on -call, part-time basis between May 3, 2010, and June 21, 2011. Our site visits were scheduled by you. At your request, CGT expanded our services to include third party observation /inspection services during the installation of soil nails within the Pump and Vault Buildings. These services were performed under the technical supervision of the Geotechnical Engineer -of- record for the soil nails and these specific buildings, Geotechnical Resources, Inc. (GRI). GRI remains as Geotechnical EoR for this portion of the project. These services were conducted on an on -call, part-time basis between August 13, 2010, and November 1, 2010. OBSERVATIONS Copies of our field reports were forwarded to our client and members of the design and construction team as the work progressed and are available in our office for your review. The following is a summary of our observations and testing services: • Architectural Wall Subgrade Soils: Based on our observations, the subgrade soils prepared for the Architectural retaining wall at the south margin of the site were prepared in general accordance with the project plans and the project geotechnical report. Carlson Geotechnical • PO Box 23814, Tigard, Oregon 97281 SW 125th Avenue Improvements Tigard, Oregon CGT Project Number G1003490 August 11, 2011 • Structural Backfill: Based on our in -place density testing, the structural fill associated with the architectural wall was placed and compacted in general accordance with our recommendations. • Sewer Trench Mitigation: As indicated in our field reports, the owner and construction team elected to mitigate the soft utility trench backfill soils within SW 125 Avenue by installing a reinforced granular mattress encompassing the trench backfill. This was presented as one of three options for mitigating the soft trench backfill in our field report dated May 3, 2010. The following summarizes our observations during construction of the reinforced granular mattress at this part of the site: o Subqrade Preparation. Based on our observations, the subgrades that we observed within the granular mattress area were prepared in general accordance with our recommendations. o Geotextile Fabric & Geo -Grid Installation. Based on our observations, the geotextile fabric and geo -grid reinforcement that we observed within the granular mattress area were prepared in general accordance with our recommendations. o Granular Mattress (Sub -base and Base Rock). Based on our observations, the granular structural fill that we observed within the granular mattress area were prepared in general accordance with our recommendations. • Asphalt Pavement: Based on the results of our in -place density testing, the asphalt evaluated within the SW 125 Avenue met required density. • Soil Nails — Installation & Load Testing: Results of our observations during soil nail installation and load testing were detailed in our field reports dated August 13, 2010, through November 1, 2010. All observations and load test results were conveyed to the Geotechnical Engineer -of- Record (EoR) for the soil nails, GRI. Based on our observations, as documented in our field reports, and to the best of our knowledge, within our scope of work, the above summaries accurately reflect our observations. Our services were provided on an on -call, part-time basis, in which we relied on the contractor to contact our office for the scheduling of our services. We cannot be held contractually responsible for those items, if any, that we did not observe or test.
