On site as requested for special inspection of epoxy. Continuously monitored the placement of
epoxied all thread. (4) 3/4x 6 %" holes were drilled into the existing stem wall (west wall). All
holes were cleaned by blowing and brushing. Simpson Set 22, ICBO #5279, batch #40634080, .
expiration date 9 -05 was used to fill holes. Work conforms to engineered drawings and
manufacturer's installation instructions.
To the best of our knowledge, items inspected on this date are in accordance with the approved
plans and specifications.
