This is not a plan review letter. Attached is a copy of the minimum requirements for a Site
Utilities Plumbing Permit. I have highlighted the required information that was not provided
with your application. Please provide the required information on the plans so that the plumbing
plan review can begin.

John Williams did a site visit and spoke with Hoa Pham, contractor, and I believe business associate with
Nelson Vial The original building is not connected to sewer at this time. Their intention is to connect and
they will connect to the private line that connects the new building to sewer service. According to John,
Mr. Pham is aware there will be additional EDU's due when they do connect. No time frames were given
as to when they intend to connect. This is all the information I have at this time, just thought I would pass
it on to you.
