As requested, Northwest Testing, Inc., (NTI) has provided construction monitoring /special inspection
of the Vial & Pham Office Building Addition. Inspections were provided on an on -call basis. Our
construction monitoring services included the following activities:
Reinforced Concrete • Reinforcement Placement
• Formwork
• Concrete Placement
• Fresh Properties Testing
• Compressive Strength Testing
A complete record of our inspection reports have been forwarded to the project team and the City of
Tigard Building Division throughout the course of this project. To the best of NTI's knowledge,
based on our inspections, the structural aspects of the project have been constructed in substantial
conformance with the approved drawings, specifications, applicable provisions of the State Building
Code, and the inspection methodology and acceptance criteria established by the project
designer(s).
