To whom it may concern:
I am granting approval for the electrical service installed by Tualatin Electric under
permit ELC 98 -1012 providing the service and all existing service disconnects are
identified and permanently plagued as required by Art. 230 -2(b) of the National Electrical
Code. Labeling must be completed and rescheduled for inspection within 20 days of this
notice. No future services may be added to building unless you comply with Art. 230 -2,
230 -71(a) and 230- 72(a).

We have received and processed your application requesting an electrical permit
for the address referenced above, and assigned a permit number as follows:

However, due to a system failure, we are unable to print the permit at this time.
We have enclosed a copy of the receipt for fees paid, along with the inspection
card to be posted at the job site. We will send you the actual permit as soon as
possible. In the meantime, you may use the above referenced permit number to
call for inspections at 639 -4175.
