This is in addition to the customers IVR number if applicable.
Terms of agreement: Accepting and performing the workauthoriaed by this work order signifies the
acceptance by the contractor of all terms and conditions set forth here in. Payment of contractors invoice
will bent 60 days upon receipt of invoice, signed customer completion section with date, store stamp,
time arrived and time completed - completed by store personnel. Labor payment is based on actual time in
and time out in fifteen minute increments, minimum charges are not accepted. DO NOT EXCEED THE
PURCHASE ORDER AMOUNT WITHOUT ACTION SERVICES GROUP WRITTEN APPROVAL
AND REVISED PURCHASE ORDER. IF TOTAL SERVICE COSTS WILL EXCEED PURCHASE
ORDER AMOUNT CONTRACTOR MUST CALL FROM SITE WITH ESTIMATE PRIOR TO
COMPLETION OF WORK OR ONLY PURCHASE ORDER AMOUNT WILL BE PAID. Contractor
must comply with all licensing, 0 ST-1A safety and any other applicable Federal, State and Lo cal laws and
regulations. Current insurance certificate must be received by Action Services Group prior to performing
this work. Contractor guarantees that his general liability and workers compensation insurances are in
effect at time of work. The contractor shall hold Action Services Group harmless with resp ect to any work
performed by contractor on Action Services Group's behalf Contractor is responsible for paying
subcontractors (if used), and up on receipt of payment of purchase order amount, releases and indemnifies
Action Services Group, and its clients, and the owner of the work location from any liens or any other
claims by himself or his subcontractor. All invoices must be received within 20 days from job completion.
