To Whom It May Concern:
Please find the enclosed attachment in reference to your building permit checklist as •
requested.
Question 5: Furnish the Building Division with a letter from the tenant clarifying •
• •Te use of the facility. To aid in the occupancy classification, demonstrate whether; -?
the patients' will be capable or incapable of self - preservation. Provide more
information , •about the treatment the patients receive, and whether the .care . r;,.
inpatient or outpatient.
• Arete Sleep Health and The Portland Clinic, intend to utilize this facility (floor •
of building) exclusively as an out - patient sleep disorders center. This involves
sleeping overnight in the facility while supervised by trained sleep
technologists. The sleep center will accept customers between the ages of 6 and
85 who are ambulatory, have the ability to sign and agree to consents for
treatment, and do not require special assistance in any way to ambulate •
themselves around the bedroom, into the restroom, and out of the facility in the
case of an emergency.
• Patients unable to move selves and/or have impaired judgment will excluded
from the services offered at this facility, and will be referred to an in- patient
environment.

• In addition to the above safety and inclusion policies, the sleep center will have
strobes in the each bedroom ceiling, mounted fire extinguishers, emergency exit
signs and maps, and emergency call cords in all patient bathrooms. The sleep
technologists are required to maintain annual CPR certification and to attend
annual mandatory ..disaster and evacuation training. As well, they receive
training in medical emergency recognition every 90 days by a board- certified
. . sleep physician.
