FIELD INSPECTION REPORT

INSPECTOR: A. Ewing — COP#499, WABO #EWI629423, OBOA #169, ICBO#1060671 -88, -85
02- 17 -05: As requested, CTI representative was on site to inspect installation of epoxy . for
various holddowns. Epoxy specified was Simpson SET, but the epoxy provided was Unitex
Pro -Poxy 300 Fast (A/B) covered by ICBO ES -5000. Per phone conversation with structural
engineer, Tim Covert, this may be a problem with respect to critical minimum edge distances
and allowable values for tension. Contractor can send product info to engineer for review or get
this specific epoxy.
Also, some holes need to be drilled deeper and longer rods provided on the main floor.
