AND CONDITIONAL CERTIFICATE OF OCCUPANCY

I, Jerry Hine, Superintendent for Sletten Companies and responsible person for the above
property, agree to the conditions set forth below and promise to fully comply with them.
This is a Temporary and Conditional Certificate of Occupancy issued today for a period
not to exceed thirty days by which time the following conditions must have been met and
approved through inspection by the City of Tigard:
All work under the following permits, including all conditions, corrections, ancillary
permits, and fee payments, must be completed and receive final inspection approval:
1. BUP2008- 00047, 3. ELR2008- 00140,
2. ELC2008- 00393, 4. ELR2008- 00141.
This supercedes and replaces the Voluntary Compliance Agreement and Temporary
Conditional and Restricted Certificate of Occupancy issued on Thursday, August 14, 2008.
I understand that with this agreement the City will withhold further legal or enforcement
action regarding these conditions until Thursday,_ September 18, 2008 and that upon
compliance with all the above conditions, this case will be closed and a permanent
Certificate of Occupancy will be issued.
I further understand that if the above conditions are not complied with fully this
Temporary and Conditional Certificate of Occupancy will become null and void at Close of
Business, Thursday, September 18, 2008, and I may thereafter be served with a Summons
and Complaint without further notice for violation of requirements of the Oregon
Structural Specialty Code and Tigard Municipal Code that Final Inspection Approval
and /or a Certificate of Occupancy be received prior to occupancy.
