FIELD INSPECTION REPORT

PROJECT: Ash Creek Condominiums — Unit 150 Remodel
ADDRESS: 11150 SW Greenburg Road — Tigard, OR

08- 03 -06: As requested, CTI representative was on site for special inspection of epoxy anchor
installation as given below.
Reference detail `B' on Tigard approved drawings "wall section ", at new foundation (at car
damaged area). Witnessed proper installation of (7) #4 rebar vertically into 6" deep holes and
evenly spaced #4 bars horizontally (extra) into stem wall, using Simpson SET PAC, lot #
093136JJ with expiration date of October 2007.
Holes were cleaned prior to installation. Installation conforms with ICC ESR -1772 and approved
plans.
