Subject: Structural Observation - FINAL REPORT
In accordance with the Oregon Structural Specialty Code, Section 1702, we have performed
structural observation for this project. We conducted site visits during the course of the
project to observe the work in progress and had verbal conversations with the contractor
and sub contractors. Structural revisions in both drawing and written form were prepared in
response to observed deficiencies and other changes. To the best of our knowledge, all
observed deficiencies and other changes have been resolved. Carlson Testing Inc. has
Issued a "Final Summary Report", dated March 29, 2004. Based on these facts, we believe
that the above referenced project structure was constructed in general compliance with the
approved plans and specifications.
