This fax is to serve as a request for extension on the Canterbury building permit - a 911 essential
facility for Washington County Communications Agency.
The extension is necessary because we have run into problems finding the approved /required
landscaping plant: prunus lusicanican. After several weeks of searching, we have been able to
track down the plant, but it is not located in the greater Portland area, and requires special
transport!
Please extend our building permit so that we might get the plant issue resolved, and finish the
landscaping, Otherwise, the site is complete.

The plan review was performed under the State of Oregon Structural Specialty Code (OSSC) 2004
edition; and the State of Oregon Fire Code (OFC) 2004 edition. The submitted plans are approved
subject to the following conditions.
1. Special inspection is required for concrete, reinforcement placement, soil compaction, bolts
installed in concrete —epoxy & high strength bolting. The special inspection agency shall
furnish inspection reports to the Engineer of Record, the General Contractor, and the City of
Tigard, Building Division, attention Hap Watkins. All discrepancies shall be brought to the
immediate attention of the general contractor for correction. The special inspector shall submit a
final signed report stating whether the work requiring special inspection was, to the best of the
inspector's knowledge, in conformance with the approved plans and specifications and the
applicable workmanship provisions of the code. 1701.3 OSSC

2. Live Loads Posted: The live loads for which each floor has been designed to exceed 50 psf shall
be conspicuously posted by the owner in that part of each story in which they apply, using durable
metal signs, and it shall be unlawful to remove or deface such notices. The occupant of the building
shall be responsible for keeping the actual load below the allowable limits. 1603.3 OSSC
3. Provide insignia of compliance information to building inspector on the equipment shelter showing
it meets the State of Oregon regulation under Oregon Revised Statue 455.705 and Oregon
Administrative Rule 918 -674. For more information please contact State of Oregon Building Codes
Division at 503.378.4133 and ask for prefabricated structures program.
Approved Plans: 1 set of approved plans, bearing the City of Tigard approval stamp, shall be
maintained on the jobsite. The plans shall be available to the Building Division inspectors throughout
