FIELD OBSERVATION REPORT

PURPOSE OF VISIT: Observe Slab Demolition
Arrived on site, as requested, to observe basement slab demolition and shallow test pit
excavations. I met with Chuck (Ken Leahy Company) and viewed demolition with him. The
floor had been demolished with a rock chipper and about 8" diameter holes from chipper were
about every 2'. The slab was fractured into pieces about every 2' to 3'.
Two shallow test pits were excavated through the demolished slab. In general I observed
approximately two feet of 4" rock overlying medium stiff silt.
I also discussed the use of material from basement walls for backfill material. We recommend
the walls be removed and hauled off site.
Based on the observations above, the demolished floor slab appears to be in general
compliance with our project memorandum dated 5/17/00.
