Building Department:
Your department told me that to decommission our septic tank, we needed to have it pumped and filled.
You further informed me that we would not need a plumbing permit but would need to bring you the
receipts for pumping and filling the tank. You referred me to Washington County to find out what fill
material to use. They said to use gravel or sand.
Enclosed are the receipts for the pumping done by A- Active on 6/7/06 and the filling with river sand by
Dave Witmore Excavating, Inc. on 6/15/06.
These receipts are to be stored in a file for this address as verification that the tank has been
decommissioned.
