As requested, Northwest Testing, Inc., (NTI) has provided construction monitoring /special
inspection of the Saier Residence. Inspections were provided on an on -call basis. Our
construction monitoring services included the following activities:
Proprietary Anchors • Material Verification
• Adhesive Anchor Installation Inspection
A complete record of our inspection reports have been forwarded to the project team throughout
the course of this project. To the best of NTI's knowledge, based on our inspections, the
structural aspects of the project have been constructed in substantial conformance with the
approved drawings, specifications, applicable provisions of the State Building Code, and the
inspection methodology and acceptance criteria established by the project designer(s).

THE FOLLOWING WAS NOTED:
NTI arrived nn'niva, as noquenmd. to monitor installation o/ and %" diameter threaded rods using Simpson SET XP and
Simpson Set 22,
1. (11) Pieces %' x 14 long all-thread rods, designated DI with embedment 9½" into exterior basement walis
(Simpson XP).
2. (4) Pieces 7/8" x 19%" long all rods, designated D2 with embedment of 15" minimum into interior load bearing
wall (Simpson Set 22).
3. (8) Pieces Y"x 18 long all-thread rods with embedment 4 minimum into (4) column bases, (2) per column, west
side of basement (Simpson Set 22)
To the best of NTI's knowledge, the anchor placement was in general conformance with project plans, specifications, and ICC
report wu.'xEGR'25O8 and E8R'1772. NTI informed Gemmell Construction personnel of alt observations.
