MTE inspector arrived on site for the epoxy dowels placement for the existing freezer
thermal break removal trench patch slab on grade at G- J/10 -11. The contractor placed
#4 rebar into 4 minimum embedment by 5/8" diameter properly cleaned holes using
sufficient Hilti HIT HY 150 epoxy, ICC #1593, lot #095622L2, expiration 7/08. Dowels in
place prior to placement of approximately 12 cubic yards of concrete.
MTE inspector also performed inspection of wedge anchor placement for the racking
systems placed at lines 9.5, 10, 10.5 & 11 /G -J (10.5 line not done). The contractor
placed Hilti KB-TZ % x 5 %" bolts, Model #387S14, ICC EST 1917, Lot #000010981938
per rack manufacturer's instructions ((2) per base post on alternate sides) and torqued to
the anchor manufacturer's specification of 40 ft -Ibs using MTE torque wrench #P267.
Holes were drilled %" diameter by 4" minimum embedment and vacuumed out
sufficiently for anchor placement.
To the best of our knowledge, items inspected this date are in accordance with approved
plans and specifications.

P.O.S. freezer racking system hold downs, Hilti KB -TZ 1 /2" x 512 ", model #387S14, ICC
ESR 1917, lot #000010981938. Wedge anchors were placed per the rack
manufacturer's instructions (2) per base post on alternate sides and torqued to the
anchors manufacturer's specification of 40 ft -Ibs using MTE torque wrench #P267.
Holes were drilled 1 /2" diameter by 4" minimum embedment and vacuumed out
sufficiently for anchor placement.

plans and specifications.
