INSPECTOR: B. Suminski COP#399, WABO #SUM522258, OBOA #340
05- 30 -00: CTI representative was onsite and noted at the basement wall line D/3 -7 a 3510B mix was placed.
Concrete tests were - performed and one -set of (4) cylinders was cast. The rebar was inspected at the A
generator enclosure footing. The rebar was placed according to plans provided. The rebar was re- inspected at
line D/7 -9 and footing line 9 /D -E. The rebar was placed according to plans provided.
