ADDRESS: 12125 SW 69th, Tigard, OR

05- 15 -00: Inspected reinforcing in #2 footing between B.5 & E.5/3 line tower crane footing,
spread footings 3B, 4B, 5F, 6F, and continuous footing D/3 -7, 9 /F -G, G/5 -9, and strap beams
5F -5G and 6F -6G. A 3000 psi concrete mix #3000B was placed by pump and concrete was
consolidated by vibrator. Observed placement of (247) cubic yards and cast three sets of (4)
6x12 samples. Reinforcing, placement, and concrete were in compliance with approved plans
and specifications.
