05- 04 -00: CTI representative was onsite and the following was noted.
1. Reviewed WPS and welder qualifications for Joe Taylor SSN# 537 -76 -7001 and William
Sasse SSN# 282 -54 -5553, with Glu Lam Structures (see attached for welder qualifications).
2. Verified torque value after installation of (6) 3 /4" diameter x 6" long Ramset wedge anchors
at 175 ft/Ibs per ICBO report #1372. Anchors were installed at grid line N3-4 roof line into
existing tilt up wall for angel iron joist ledger. Two wedge anchors, one at each ledger plate
were unable to meet required torque value. Re- inspection to follow once re- installed. All
anchors achieved 4" minimum embedment per detail 5 of fax received from Jim
Riemenschneider with VLMK Engineering (see attached).

Here's what needs to be done to reinforce the roof per the meeting on May 2nd,
for building A, under the new mechanical unit
PAY SPECIAL ATTENTION TO THE LOCATION OF THE UNIT AND THE
CURB OUTLINE. THE CURB MUST No EXTEND SOUTH OF THE
EXISTING JOIST THAT IS 10' NORTH OF GRID "3 ", NOR EXTEND
NORTH OF THE EXISTING JOIST THAT IS 10' NORTH OF GRID "4 ".
