On May 20, 2009, UL conducted a Field Evaluation on the above subject equipment. We then sent you a preliminary
report on 08/03/2009, which detailed non - compliant issues. It has now been 3 months since our initial evaluation.
This project has remained open awaiting communication from you that the items noted in the preliminary report have
been corrected. To date, we have not been able to establish the completion schedule. Since a firm schedule for the
completion of the Field Evaluation cannot be established at this time, we are closing this project as of this date and
notifying our accounting department to invoice you for the charges incurred.
In the future, once you are ready for the completion of the Field Evaluation, please contact us at 1- 877 -UL -HELPS
(prompt #2) and we will open a new project and schedule a date to complete the Field Evaluation. If you believe
there is an error in the above action or if you have any questions, please do not hesitate to contact me.
