FIELD INSPECTION REPORT

PROJECT: Just Sports @ Washington Square — Change of Occupancy
ADDRESS: 09714 SW Washington Square Rd — Tigard, OR
INSPECTOR: R. Andree — COP #763, ACI #048632, OSHD #42872
09- 30 -08: At the request of Jim with Commercial Building, CTI representative did pull tests on
(2) 5/8" 1 wedge anchors. CTI representative did 2 pull tests at center of main columns at
storefront. Two sacrificial anchors were set & CTI representative used a 12 ton ram & 1500 /psi
gauge to load each anchor to 2000 lbs. as requested by engineer. Each anchor held load
without movement.
