Report (698) 134 Pam Oo) r Structural Observations Report Final Inspection of Structural Work Project: Durham WWTP Crystallizer Installation Structure Name: Influent Pump Station Observation Performed By: Matthew Smith /CH2M HILL Date of Visit: April 1, 2009 Report Summary: On Wednesday, April 1, 2009, the final inspection of the Durham WWTP structural work was performed. With the exception of the final paint finishes, the structural work appeared complete and matched the structural drawings. Only work relating to the CH2M HILL contract was inspected and systems designed by others were not reviewed. The following outlines the structural observations: Highlights: Doorway Support Column: During the course of the reactor installation, the HSS column supporting the doorway skin framing and hoist beam was removed and reattached. (Reference RFI M -013) The final condition of the column could not be verified because at the time of the visit, the skin framing was back in place. However, after review of photographs taken by the client and contractor as well as the visual inspection report of Carlson Testing, it appears that the column was adequately welded back into position. See attached photo, PL -1. Reactor Frames: All three OSTARA reactors were in place and filed to standard operating levels. The connections appeared to match the contract drawings and the welds on the modified brace connections appeared adequate except that the final finish had not been applied. The grout bearing pads looked to be adequately formed and the frames seemed level. The modified Reactor /Frame connections were installed, but some were not painted. See attached photos, PL -2. Steel Floor Beam: The steel floor beam supporting the control room structural slab appeared complete. The anchor bolts matched the structural drawings and the grout appeared to be in place. See attached photos, PL -3. Concrete Opening Infill: The concrete infill repairs of the openings in the Motor Room slab were complete. The finish nearly matched the existing slabs. Several pop outs were visible from underneath, but the structural integrity does not appear to be impacted. It was noticed that some of the form holes were not completely filled with grout. See attached photos, PL -4. Steel Column: The steel column located in the pump room was in place. The grout bearing pad appeared to be properly formed and the anchors bolts properly positioned.
