On this date a PS1 representative; \ ; rs a the above referenced site to perform 'special inspections as requested or
required in accordance with job sr :cifications, IBC, and applicable codes.
Location: Ale mezzanine column ' qtse anchor bolt and stairway bottom anchor bolts.
On site to monitor the install of of expansion anchors per iBC code, manufacturer and design specification
requirements. •
Monitored the installation of 3c e .esh 5/8 "x6" and two each '4 "x5 'A" Simpson expansion anchors at the above noted
location. Verified clean holes ;nt ■limensions (1/2" and 5/8" diameters (cal due date 03/ and 4 " d ). Also

60 and 90 ft/lbs torque using torq i.: wrench S/N 3060713238 ( Equipment P ment p
FMTIC'r drawing # 09119 -01 for i ;rtallation details and information.
Verified that 5/8" and '1/4" grade 5 beam to beam to column bolts are installed and tensioned to snug tight condition.
To the best of my knowledge, tae work was in accordance with the building department approved design drawings,
specifications, and applicable v tmanship provisions of the IBC.
