Contractor is hereby authorized by the City of Tigard to perform CONTRACTOR
the additional work described below in accordance with the terms
and conditions detailed in the original contract along with all
applicable rules,regulations,and laws that may be in effect for the Signature
work. The unit pricing in the original contract shall apply to all
additional work. A copy of this form, once completed,is to be
forwarded to the Purchasing Office to ensure all changes to the
encumbrances are met. Remember— the cumulative total of

AMENDMENT #1
The Agreement between the City of Tigard,a municipal corporation of the State of Oregon,hereinafter called
City, and FieldTurf, hereinafter referred to as Contractor, entered into on the 20' day of January, 2021, is
hereby amended as follows:
9. Compensation
A. Progress Payments: City agrees to pay Contractor Four Hundred Six Thousand Five Hundred
Seventy-Seven and No/100 Dollars ($406,577.00)
for performance of those services
provided as the revised "Base Bid" and "ADD Alternate #3" hereunder. whieh p Payment
shall be based upon the following applicable terms::

SCOPE OF SERVICES
For ease in understanding change to the Turf Infields and Fencing of this project,while similar to the original
scope, the parties shall strike the full Civil Services items in the Original Contract and replace with the
following:
BASE PROJECT—Cook Park Little League Infields
Turf System : XT-50, Pile Height: 2", Infill Weight: 5 lbs/sq. ft., Pile Weight: 30 oz./yd2, Total System
Weight: 773 oz./yd2
North Field— 11,221 SF
South Field—9,092 SF
ADD Alternate #3—Fencing
Furnish and install black powdercoated posts and fencing in lieu of galvanized option.
Services under this work include:
1. 3rd Party Design Services and construction documentation including:
a. Review existing Geotechnical information and Stormwater Calculations prepared by others
b. Layout Plan
c. Grading Plan
d. Utility Plan
e. Field Marking Plan
f. Detail Plans to include curbs, piping systems, base section and additional details as required to
complete the installation of the field
g. Furnish Sealed Drawings
2. General Conditions including:
a. Construction Staking
b. Temporary Fencing
c. Temporary Toilet
d. Construction Plating and Entrance
3. Mobilization
4. Survey and Layout of our work
5. Cut& cap existing irrigation
6. Mass excavation and haul spoils off site as required to meet subgrade elevations
7. Laser grade and compact subgrade
8. Furnish and install reinforced concrete curb with floating 2x4 nailer detail at outfield arc
9. Furnish and install geotextile fabric at the turf limits
10. Furnish and install 2 x 4 treated wood nailer at perimeter curb
11. Furnish and install 8" perforated perimeter pipe and tie-in within 20 LF of turf limits
12. Furnish and install flat panel drains
13. Furnish and install 4" base stone,laser and compacted
14. Furnish and install 2" finish stone,laser and compacted
15. Furnish and install (1) set of sleeves/ anchors, bases, home plate and pitcher's rubber,per field
16. Furnish and install 24' high backstops. 18' netting on top of 6' high chain link. Backstop dimensions at
appx 42'x 17'x 42'. Backstop posts to be 4" sch 40 with 6' set.
17. Supply and install appx 618 linear feet of 6' high fencing. Fencing to include: 2 7/8" terminal posts, 2
3/8" line posts, 1 5/8" top and bottom rail.
18. Supply and install four 4'x 6'man gates.
19. NOTE—all backstop and fencing to be galvanized
20. Site restoration at disturbed areas
21. One-year warranty on civil work
22. Installation of the artificial in-filled grass surface upon a suitable base
23. Installation of inlaid markings as submitted
24. Prevailing wages
25. A 5 year 3rd party pre-paid insured warranty on the FieldTurf artificial grass surface; with exception on
the home plate area,base paths and pitcher's mound which shall be 2 years instead of 5 years and there is
no third party insurance.

• Price based on application of Portland Cement. 5-6 lbs. per SF 12" deep (5-6% by weight @
1-lbs dry density PCF) based on field conditions and onsite recommendations from Geotech.
• Single Lift Subgrade - One Spread/One Mix.Approx. 22,000 SF
• Spread/Mix at a minimum width of 8' is anticipated.
Cement Amendment#2- Cook Park Baseball Field Improvements per Carlson Geotechincal Report
dated March 10, 2021, specifically limited too:
• Price based on application of Portland Cement. 8 lbs per SF, tilled 18" deep (as directed by
Carlson Geotechnical).
• Single Lift Subgrade - One Spread/One Mix.Approx. 20,000 SF
• Spread/Mix at a minimum width of 8' is anticipated.
