As you know, the investigation was conducted at the request of both Andy Ewing of Carlson Testing and Jeanine F ylingness of Windrnar Development. They both hove expressed concern over the construction of Building "A," in particular. Once we visited the job site, we observed the following items on Building "A"; which include Andy Ewing's concerns. I I) Beam to column connections not interfacing for proper bolt installations. 2) Bracing connection tees not cut on the proper angle for uniform bearing with beams. 3) Beam flange cut for partial penetration field welding as opposed to required full penetration welding. These beams must be removed and re-ground. 4) Beam to column connection has bolt holes field cut with torch which are unacceptable. 5) Bracing gussets have no holes for accepting angle bracing (col. liner D & G). 6) Colur.m base plate has oversized holes for anchor bolts which were mislocated. 7) There are many cases where the anchor bolts ere missing, mislocated or there are the wrong number of bolts. 9) Beam to column clear distance is greater than specified and noN requ!res backing bars for full penetration welds. 700 Iloyci bldg,st,at le. wo 98101 (200 622 5822 anchorage las angelee partlar.d Seattle M --....------.—.EJ Mr. Walt Schneider December 8, 1983 Page 2 9) Some column base plates are set directly on concrete with no provision for grout space. 10) A recent survey indicates some columns are mislocated. II) At least one beam was cut shorter in the field to fit between the columns which were actually mislocated. This beam will probably have to he replaced when the column; are relocated. 12) Glue-larn hinge connectors do not have 'iertically sl-,tted holes, and top straps are placed backwards. :3) Relocation of columns will require expansion anchor bolt installation, along with expansive grout under plate for uniform bearing. 14) Glue-lam rafter bears seat has unacceptable seam weld and must now be welded on the inside with glue-lam shaped to fit for uniform bearing. 15) Beam has gusset plate missing for knee hrace. 16) Some beam connections were installed at the incorrect elevations and have now been relocated. Some have yet to be corrected. 17) One of the sloping ueums on line G was fabricated without the proper bevel on one end and will require a special connection to he designed if left as is. 1 18) Most of the top level sloping beams on lines D & C do not fit properly and will require a modified connection to be designed. 1°) There .s a missing horij'ontal stiffener on the glue-lam beam seat or, column D-4. 20) There are certain areas of columns where primer is missing or scaling off and i equi res rework. In summary, the above items seem to fall into three categories of problems. First, the dimensional control of this project has been inadequate which has led to foundations being installed in the wrong locations. Secondly, the foundation construction has been poorly controlled wh.,:h has resulted in many footings which hove been constructed at the improper elevation or that have anchor bolts improperly installed to fit the coli ,ins. Thirdly, the structural steel in many instances has been either dimensionally mistabricated or fabricated not in uccordance with the structural drawings which we feel is partially due to inadequately detailed shop drawings. The aforementioned items have caused delay to the construction schedule and are a continuation of similar problems that occurred on Building "C." rt Mr. Walt Schneider December 8, 983 Page 3 It has been determined " will be necessary to relocate some of the columns in Building A to properly cor,_'.,.-t this building. We recommend that lb.. building columns be located with resnect to tl.e north CMU wall which has been constructed. Many of the obeve items will require further engineering analysis to confirm the adequacy of their present condition. In addition, the Washington County Building Officials are now requiring that all deviations from, the structural drawings be verified by details and calculations from tie structural engineer. If it is decided to continue constructing this building not in accordance with the details on the structural drawings, substunticl additional engineering effort will he required to verify the structural integrity of the building. We would, therefore, like to receive direction to proceed re engineering these field changes or reject these conditions and require conformance with the original contract drawings. In addition, we would strongly recommend that properly detailed shop drawings for the structural steel be submitted in conformance with industry s andard practice, for building "B", prior to fabrication. These draft stop partitions may be constructed of 1/2-inch-thick gypsum plasterboard, 3/8-inch-thick plywood or uther material approved by the Fire Marshal . Combustible concealed spaces if created in the over- hanging roof area must be subdivided by draft stops at maximum intervals of 100 lineal feet. (ref: Sec. 2516 U.B.C. ) 3. Parking spaces provided for use by the physically handicapped must be identified by a sign centered at least 4 feet above grade, near the required perking space. The sign shall he marked with the International Symbol of Access and shall have the following words in at least 10 inch high letters. "Parking by DMV Disabled Permit only. Violators subject to towing under ORS 487.535 and a fine up to $250 under ORS 487.930. " Reference Section 3108, SBC. 2. At least one accessible route designed for use by the physically handicapped moist be provided from Public Transportation stops, designated parking and passenger loading zones, public streets or sidewalks to the building entrance. At least one accessible route shall connect buildings, f,.cilities, and spaces that are on the same rite. 3. The 1980 ANSI A117.1 Standards are adopted by reference as part of Chapter 31 of the 1982 State Building Code. The standards shall apply to all Government or public buldings as defined in Section 3103, SBC. 4. Standards require that toilet compartments fcr the handicapped have a minimum width of 60 inches and a minimum length of 59 inches when provided with floor mounted water closets. Grab bars must be mounted at a height of 33 to 36 inches above the floor. Urinals shall be stall type or wall-hung with an elongated rim at a maximum of 17 inches above the floor. 5. Assembly type occupancies such as drinking and dining establishments, arcades or similar tenant spaces, that may be developed in the project, now or in the futu;'e, which require toilet facilities for other than employees by Table 5-E will require that each such tenant also provide facilities designed for use by the physically handicapped. 6. Glass u.,ed in doors or in glazing located with 12 inches of the nearest vertical edge of a door or in areas subject to human impact, shall be tempered, Reference Section 5406, SBC. 7. Enclosed attic spaces formed of combustible construction shall be divided into horizontal areas not exceeding 9000 square feet by partitions extending from the ceiling to the roof. Such partitions shall be not less than 1/2 inch thick gypsum wallboard, or 1 inch nominal thickness thigt-fitting wood, 3/b inch thick plywood, or approved non- combustible material dequately supported. Openings in the partitions shall be protected by self-closing doors constructed as required for the partitions. Reference Section 3205, SBC. b. This office will accept the stamp of the architect on the plans of records as certification that to the best of his professional knowledge, information and belief, that the plans are in conformance with the requirements for energy conservation of reference Section 5301 , SBC. 9. All insulation materials including facings, such as vapor barriers or breather papers installed within floor-ceiling assemblies, roof-ceiling assemblies, walls, crawl spaces or attics shall have a flame-spread rating not to exceed 25 and a smoke density not to exceet 450 wheo tested in accordance with (IBC Standards No. 42-1 . I 1 . Automatic sprinkler protection must be installed under the overhanging roof area. Heads should be provided at maximum 15-foot intervals and we would suggest the Pmployment of dry-type sidewall sprinklers for this purpose. Sprinklers may be emitted under the overhanging roof spaces where masonry block walls occur. However, storefront and combustible wall spaces under the eaves must be provided with sprinkler coverage. 2. The main drain must be piped to spill to t',e outside in an area where full flow tests of the water supply may be made at normal operating pressures without causing water damage to the building, the surrounding landscaping or to vehicles in the parking area. Spilling the main drain into an open hub is net acceptable. 3. An inspector's test facility must be provided. It lust consist of a smooth bore corrosion-resistant fitting with control valve which will provide a flow equivalent to one sprinkler head. In the first instance, in all wood framed walls and partitions fire- stopping is required atall floor and ceiling lines and in the case of double stud walls and partitions at maximum horizontal intervals of 10 feet. That is, concealed combustible spaces must be fire- stopped so that no dimension exceeds 10 feet without d barrier to the passage of flame. Penetrations made in this prescribed firest. piog to accommodate wiring, duct work, plumbing and similar utility runs must be packed, following the installation of said utility runs, in a manner which will effectively maintain the integrity of the firestopping aed cut off all concealed draft openings which could afford a passage for flame. The doors, shown on the drawings, must be openable from the inside it all times for immediate exit without the use of a key, special know- ledge or effort. Further, a floor or landing must be provided on each side of every doorway and this floor or landing must riot be more than 1/2-inch lower than Cie threshold of the doorway. A firefighting water supply having a minimum aggregate flow of 1 ,250 g.p.m. at a minimum residual pressure of 20 p.s. i . from standard fire hydrants located within 250 feet of all portions of this building is required. Washington Square Road This letter relates primarily to the plans for the installation of automatic sprinkler protection in Building A, but some of our comments apply to all three of the structures which compose this complex. In any case, we have ex- amined the plans in conjunction with an on-site inspection of the actual in- stallation and as a result offer the following comments. r 1 . At grid lines J-K/5, the wet pipe branch line in which three automatic sprinklers are shown may be replaced with one drystem sidewall head as i have shown on the drawings in red. That is, the three sprinkler heads under discussion are located in a space which is subject to freezing and no provisions have been made to provide heat in this area. Accordingly, , to prevent freezing of this portion of the system and to provide at least minimal coverage in the space, a dry sidewall head nay be installed in this i- stance. This stipulation also applies in the similar cases which occur in buildings B and C. 2. From observation at the floor line it appears that there are a number of branch lines which cannot be made to drain through the main drain. All parts of the system must he provided with drainage and where the system cannot drain through the main drain, auxiliary drains must he provided. Where the capacity of trapped sections of pipe ii 5 gallons or less, the auxiliary drain may consist of a one inch nipple and cap or brass plug. When the capacity of trapped sections of pipe is more than 5 gallons, the auxiliary diain must consist of a one inch valve complete with a nipple and cap or brass plug. $TOT,PES SAVES LIVES Mr. Jerry Foley March 23, l'.34 Page 2 You will recall that we discussed this issue by telephone some time ago and it is understood that you will make suitable provision for the com- plete drainage of the entre system, not only in Building A, but in buildings B and C as well . 3. As mentioned in my letter to you of January 23, covering the sprinkler protection drawings for Building C. the main drain must not be piped to ' spill into an open hub. The reason for this is that the main drain is used during our routine fire prevention inspections to test the water supply to the automatic sprinkler systems. That is, the main drain is open and is operated for a substantial period at normal operating pres- sure so that we are assured that the water supply has not been rendered deficient through work conducted on the public water supply. Open hub and similar types of drains are not capable of handling the normal flow thru the main drain at the normal system pressure. Thus, water damage results. Accordingly, the main drain must be piped to spill to the outside in an area where full flow tests may be made at normal operating pressures without causing water damage to the building, the surrounding landscaping or the vehicles or anything else parked within the are. If this require- ment is nut fulfilled, the system will not be approved and occupancy of the building will .iot be permitted. Again, this requirement ar.plies to all three structures. 4. The system must be fully and electrically supervised and must he connected to the fire alarm center of Washington County Fire District No. 1 or to an approved central station; e.g. Honeywell , ADT, etc. Subject to the foregoing stipulations the plans are hereby aprroved. We have retained one copy for our records. The remaining copies and hydraulic calcula- tions are herewith returned to you. ( I Re: Square Too Confirming our telephone conversation of June 12 1 have examined the tenant improvement plans for the paper station (PC 4069) in Building 8, Portland Luggage (PC 4070) in Building A, City Kids (PC 4071 ) in Building A, Osborne and Ulland (PC 4072) in Building A and the Compu- shop (PC 4073) in Building C and in each instance there are problems with the proposal as regards tire safety regulations. In all cases the potential occupant load based upon floor area as prescribed in the regulations of this jurisdiction exceeds 50. As result, a minimum of two exits are required and access to these exits must not pass through stock or storage rooms. Further, they may not pass through kitchens, restrooms, closets or spaces used for similar purposes. Also, door assemblies which serve a potential occupant load of 50 or more must be hinged to swing outward in the direction of xit travel and a floor or landing must he provided on each side of the door opening. This floor or landing must be not more than 1/2-inch lower than the is threshold and where doors swing outward over l,ndings the landing must have a minimum length of 5 feet I am sorry that the issue of 5-foot landings has apparently come as a shock to you. Still , your architert, your building contractor and your firm was infcrmed of this requirement in our Plans Examination Report No. 1504 and dated August 25, 1983; well before construction began and thus in ample time to make adequate provisions in the construction documents. In any case, we cannot approve the plans as STOP FIDES --SAVES(IVES Ms. Jeaniene Fyl1ingness June 13, 1984 Page 2 submitted nor endorse the building permit until the foregoing issues are satisfactorily resolved. Other requirements which will ultimately come into play in the course of developing the three buildings which compose Square Too are the extension of sprinkler coverage to the extent that all portions of every occupancy are fully protected. Also, attic draft stops must be provided to the effect that the maximum dimension of any attic space is 100 lineal feet. Care must be taken in laying out the attic draft stop locations to avoid interference with the automatic sprinkler coverage in the roof area. Please alter the tenant improvement plans under discussion to accommodate the foregoing requirements and then resubmit for a building permit. What this means is that in individual stores exit signs may be illumi- nated by the normal store lighting fixtures and need not be internally illuminated. That is, the regular store light may shine on the exit signs and no special electrical features are needed in such cases. However, the ways of exit, exclusive of the main entrance, must be indicated by means of exit signs with the letters being not less than 6 inches in height and the principal strokes being not less than 3/4 inch in width. On the c her hand and with special emphasis on Building C which has in internal corridor system serving many tenant spaces and therefore a potential occupant load of over 300, exit signs must be electrically illuminated by not less than. two (2) lamps of 15-watt minimum capa- city. One lamp may be served by the regular illumination circuit which serves the remainder of the building while the second lamp must be served either by an emergency circuit or by an emergency power suppl;. Further, the corridor must be illuminated to a value of 1 foot4•andie measured at the floor and this illumination must be by means o' an emergency circuit or an emergency power source. In addition, we would accept 3/4 inch Lag Bolts, lagged down from the top in place of rising vertical through bolts. 2) As requested, we are forwarding the equivalent staples that may be used in place of nails. On the roof diaphragms, Senco No. 14 Ga. Staples x 1-3/4 inch may be substituted for the 8d common oails specified provided that +he same spacing is maintained. (See attach 30 approval.) 3) As requested, we are forwarding the equivalent staples that may be used in place of IOd nails, on the shear walls. We recommend using Senco leo. 14 Ga. x 1-3/4 inch staples with a decrease in spacing of 25%. In other words, 4 inch o.c. as now shown on The drawings will be 3-1/4 inch o.c. 4) Another request was to use 7d nails at a closer spacing as opposed to 8(1, 7d common nails are acceptable if they decrease the spacing by 10%. (See attached calculations.) 5) The shear w al on line 4 was modified due to height restriction. We have approved dropping of the top plates, provided the plywood laps the glue lam. This will restore the top chord in effect and work just fine. 6) We inspected thr connection on the southeast corner of building "C" and gave our recommendations. As you know, the Building Department and Carlson Inspection and Testing Agency has expressed concern over a series of items on the building "C" structure. Once we obtained your approval on the field visit, we met with Roger Simonotti of Westwood Construction and Andy Ewing of Carlson Inspection and Testing Agency. We reviewed in detail the following items: I) Beams fabricated too short. 2) Welding knee brace connections as opposed to specified bolting. 3,4) Welding X-braced frames to replace bolted connections. 5) Welding to replace bolts on framed beam connections. 6,7,8) Anchor bolt conditions with insufficient projections. 9,10) Glu-lam hanger welding inconsistencies. In completing our field inspection, we indicated a number of corrections had to he completed. These corrections could only be accomplished after we've hod a chance to review the conditions and redesign, if deemed necessary. We hove now completed our analysis and would like to make the following r ecommendations: Item No. I) Beams on line A of columns 6 and 7 cre too short by 2 inches. Correct by installing new shear plates. (See detail I, attached.) 2) Knee braces on line 4 at columns 6 and 7 -equire welding to replace three halts. (See detail 2, similarly attached.) 4) Welding required to replace bolts from X-braced connection Tee's to top of W8x24's on line 4. (See detail 3, attached.) 5) Welding required to replace bolts on double sheir connections for W 14x26 beams (sloping) on line 4. (See detail 4, attached.) 6) Welding required for anchor bc!ts projecting above base plate 1/4 inch or more. (See detail 5, attached.) 7) Anchor bolts with no projection above base plate that have been welded without inspection require additional anchor bolts. Provide two 3/4 inch diameter red head expansion bolts or equal on all base plates where two ur more anchor bolts are plug welded. (See detail 6, attached.) 8) Anchor bolts in 21 inch column base plates on line 4 that are missing or in wrong location require the following: a) Remove expansion bolts in plate holes along with all spauleu concrete. b) Install Embecco 602 non-shrink grout in drilled expansion bolt holes and under base plate. c) After grout has cured. redrill holes and install 3/4 inch diameter expansion bolts extending of least 5 inches below spouted concrete surface. 1 d) Leave temporary bolts and angles in place. 9) Glue laminated beam hangers require inspection by Carlson Inspection and Testing. Weld thicknesses on a spot check of steel plates is riot in conformance with approved drawings. The two-sided hanger back plate (section G S-i8) requires a minimum of 3/8 inch fillet weld to the cap plate. 10) The single-sided glue laminated beam hanger must have at least a 3/4 inch thick top plate welded to the back plate with a 3/8 inch rillet weld. As you know, we exprer':ed our concerns earlier on the lock of details, dimensions, and piece marks on the shop drawing. It's cur op;pion that most of these items did not fit in the field because of this problem. Based upon our inspection of the welds, we would highly recommend that all welds be inspected by the inspection and testing agency, which includes all shop and field welds.
