Per our phone conversation, I have created fee estimates for the Compass Oncology site work permit and medical vault
building permit with their respective valuations below. The plan review fees listed below will be due upon
submittal. You are welcome to pay this fee in person or online after submitting. Also, I did not include the archiving
fees, which are $.50 per page for pages up to 11" x 17" and $2.00 per page for pages over 11" x 17 for one complete set
of plans. Furthermore, as we discussed, we will be open on the December 26 and 27. Please let me know if you have
questions.

Hi Bruce,
I have attached the receipts for the Compass Oncology site work and building permit that you submitted earlier. We
were fairly busy after you left, which is why it took longer to send these to you. Also, I confirmed today that Fire and Life
Safety plan review would need to be collected for the medical vault building permit. My estimate for the building permit
did not include this fee. I am certain I verified this before the estimate with our plans examiner, but either way, we will
need to collect this fee once the permit is ready to issue. Fire and Life Safety plan review will be$2,697.98 based on the
valuation of$1,200,000.00. I apologize for the inconvenience. Please let me know if you have questions.
