9. HOURS OF CONSTRUCTION ON COLLECTOR AND ARTERIAL ROADS
WILL BE LIMITED TO 9:00 A.M. - 3:00 P.M. UNLESS AUTHORIZED BY
THE CITY ENGINEER. NO WORK WILL BE PERMITTED ON
COLLECTOR AND ARTERIAL ROADS BETWEEN 3:30 P.M. AND 9:00

10. MINIMUM TRAVEL LANE WIDTH SHALL BE TWELVE (12) FEET;

11. THE CITY RESERVES THE RIGHT TO ADD TO OR MODIFY TRAFFIC

13. THE PERMIT HOLDER OR HIS CONTRACTOR SHALL NOTIFY THE
CITY'S INSPECTOR AT 503/639-4171 TWENTY-FOUR (24) HOURS
PRIOR TO COMMENCING WORK, TWENTY-FOUR (24) HOURS PRIOR
TO ANY STAGED INSPECTION AND AFTER COMPLETING WORK
COVERED BY THE PERMIT.
14. A COPY OF THE PERMIT AND ALL ATTACHMENTS, AND A COPY OF
THE APPROVED CONSTRUCTION PLAN AND ALL AMENDMENTS
SHALL BE AVAILABLE AT THE WORK AREA. ALL WORK SHALL
CONFORM TO THE PERMIT TERMS, CONDITIONS AND PROVISIONS
AND TO THE CITY APPROVED PERMIT PLANS, AND APPROVED PLAN
AMENDMENTS AND TO THE CITY'S STANDARDS AND
SPECIFICATIONS AND TO THESE GENERAL CONDITIONS. CHANGES
TO ANY OF THE AFORESAID MUST BE APPROVED BY THE CITY, IN
ADVANCE OF WORK PERFORMANCE.
15. MAINTENANCE OF THE WORK AREA AND APPROACH ROADS IS THE
RESPONSIBILITY OF THE PERMIT HOLDER. THE WORK AREA AND
APPROACH ROADS SHALL BE MAINTAINED IN A CLEAN CONDITION,
FREE FROM OBSTRUCTIONS AND HAZARDS. A COPY OF THE
PERMIT HOLDERS CERTIFICATE OF INSURANCE SHALL BE
AVAILABLE AT THE WORK AREA.
16. THE SPREADING OF MUD OR DEBRIS OR STORAGE OF MATERIALS
OR EQUIPMENT OF ANY KIND UPON ANY PUBLIC ROADWAY IS
STRICTLY PROHIBITED AND VIOLATION SHALL BE CAUSE FOR
IMMEDIATE CANCELLATION OF THE PERMIT. THE CITY MAY AT ANY
TIME ORDER IMMEDIATE CLEAN UP AND STOPPAGE OF WORK TO
ACCOMPLISH CLEAN UP.
17. EFFECTIVE EROSION CONTROL IS REQUIRED. EROSION CONTROL
DEVICES MUST BE INSTALLED AND MAINTAINED MEETING THE
CLEAN WATER SERVICESD AND D.E.Q. REQUIREMENTS. THE CITY
MAY AT ANY TIME ORDER CORRECTIVE ACTION AND STOPPAGE OF
WORK TO ACCOMPLISH EFFECTIVE EROSION CONTROL.
18. PROPERTY DISTURBED BY CONSTRUCTION ACTIVITY SHALL BE
SEEDED WITH A STANDARD GRASS MIX; SHRUBS, FLOWERS,
BARKDUST, EXISTING SIGNS, PAVEMENT MARKINGS, MAILBOXES,
ETC. SHALL BE REESTABLISHED, REINSTALLED OR REPLACED,
WITH LIKE KIND AND MATERIAL.
19. EFFECTIVE DRAINAGE CONTROL IS REQUIRED. DRAINAGE SHALL
BE CONTROLLED WITHIN THE WORK SITE AND SHALL BE SO
ROUTED THAT ADJACENT PRIVATE PROPERTY, PUBLIC PROPERTY
AND THE RECEIVING SYSTEM IS NOT ADVERSELY IMPACTED. THE
CITY MAY AT ANY TIME ORDER CORRECTIVE ACTION AND STOPPAGE
OF WORK TO ACCOMPLISH EFFECTIVE DRAINAGE CONTROL.
20. OREGON LAW REQUIRES FOLLOWING THE RULES ADOPTED BY THE
OREGON UTILITY NOTIFICATION CENTER. SAID RULES ARE SET
FORTH IN OAR 952-001-0010 THROUGH OAR 952-001-0080.
COPIES OF SAID RULES MAY BE OBTAINED FROM THE CENTER BY
CALLING 503-246-1987. IF YOU HAVE ANY QUESTIONS ABOUT THE

EXCAVATION & GRADING:
1. THE CONTRACTOR SHALL DETERMINE THE TYPE OF EQUIPMENT AND
METHOD TO USE TO ACHIEVE REQUIRED COMPACTION. THE
CONTRACTOR SHALL ARRANGE FOR A GEOTECHNICAL ENGINEER TO
TEST AND CERTIFY SOIL COMPACTION. THE CITY OF TIGARD, AND THE
ENGINEER SHALL RECEIVE COPIES OF ALL GEOTECHNICAL INSPECTION
AND TEST REPORTS.
2. PIPE BEDDING SHALL CONFORM TO THE GRANULAR BEDDING AND
BACKFILL REQUIREMENTS OF CWS PER DWG NO. 590. BEDDING SHALL
BE 3/4"-0" CRUSHED ROCK, EXCEPT WHERE NATIVE MATERIAL MAY BE
USED.
3. TRENCH BACKFILL WITHIN THE RIGHT-OF-WAY SHALL BE CLEAN,
WELL-GRADED 3/4"-O" CRUSHED ROCK COMPACTED TO 95% AASHTO
T-99.
4. ALL AREAS OF FILL THAT WILL BE WITHIN A FUTURE BUILDING ENVELOPE
SHALL BE COMPACTED IN ACCORDANCE WITH THE UBC, CHAPTER 33.
5. THE CONTRACTOR SHALL PROVIDE PROTECTION TO ADJOINING
PROPERTY FROM EXCAVATION AND FILL ACTIVITIES AND FROM
SEDIMENTATION DUE TO RUNOFF. THIS MAY BE ACCOMPLISHED BY THE
INSTALLATION OF APPROPRIATE DRAINAGE DITCHES NEAR THE
PROPERTY BOUNDARIES AND BY KEEPING GRADING ACTIVITIES AT
LEAST 2 FEET AWAY FROM PROPERTY BOUNDARIES, AS REQUIRED BY
THE UBC CHAPTER 33.
6. TRENCHES ARE NOT TO BE LEFT OPEN OVERNIGHT.
ROADS:
1. MATERIAL IN SOFT SPOTS WITHIN THE ROADWAY SHALL BE REMOVED
TO THE DEPTH REQUIRED TO PROVIDE A FIRM FOUNDATION AND SHALL
BE REPLACED WITH PIT RUN CRUSHED ROCK. THE ENTIRE SUBGRADE
SHALL BE THOROUGHLY COMPACTED TO A RELATIVE DENSITY OF 95%
OF T-99. 4" ROCK IS THE MAXIMUM ALLOWABLE SIZE.
2. WET WEATHER CONSTRUCTION AND OTHER UNFORESEEN CONDITIONS
MAY REQUIRE AN ADDITIONAL ROCK SECTION AND THAT GEOTEXTILE
FABRIC BE PLACED ON THE SUBGRADE. IN THIS EVENT, THE
CONTRACTOR IS TO CONSULT WITH THE ENGINEER AND WORK TO THE
WRITTEN RECOMMENDATIONS OF THE ENGINEER.
3. THE CONTRACTOR SHALL NOTIFY THE CITY OF TIGARD FOR INSPECTIONS
(1) WHEN THE SUBGRADE IS COMPLETE, (2) 24 HOURS PRIOR TO
PLACEMENT OF ROCK BASE MATERIAL AND (3) 24 HOURS PRIOR TO
FINAL PAVING FOR AN INSPECTION OF THE WORK, AND AT PROOF-ROLL
FOR CURB AND GUTTER AND FINAL PROOF-ROLL PRIOR TO PAVING.
4. THE ROADWAY MUST BE PROOF-ROLLED UNDER INSPECTION OF THE
ENGINEER AND THE CITY OF TIGARD WHEN THE SUBGRADE IS
COMPLETE AND WHEN THE TOP ROCK IS INSTALLED PRIOR TO PAVING.
5. THE ASPHALT CONCRETE PAVEMENT SHALL BE PROVIDED FROM A MIX
FORMULA AS SPECIFIED BY ODOT SPECIFICATION SECTION 00744
(MINOR HOT MIX) , LEVEL II 1/2" HMAC OR WMAC. COMPACT AC
PAVEMENT TO MINIMUM 92% OF MAX. DENSITY PER RICE STANDARD
METHOD. THE CONTRACTOR IS TO PROVIDE THE ENGINEER WITH
CERTIFICATE OF COMPLIANCE FROM THE ASPHALT PAVEMENT PLANT
UNLESS OTHERWISE INDICATED.
6. THE DEVELOPER SHALL INSTALL STREET LIGHTS PER THE INLCUDED
LIGHTING PLAN.
STORM DRAINAGE:
1. THE CONTRACTOR SHALL USE PIPE MATERIALS AS DESCRIBED ON
PLANS. ALL CHANGES NEED TO BE APPROVED BY THE CITY PRIOR TO
CWS. IF NEEDED, THE CITY WILL SEND TO CWS FOR APPROVAL.
2. ACCEPTABLE STORM DRAIN MATERIALS ARE AS SHOWN ON PLAN AND
PROFILE SHEETS AND SHALL BE TESTED IN ACCORDANCE WITH C.W.S.
STANDARD SPECIFICATIONS. (R&O - 07-20)
3. STORM DRAIN PIPES SHALL BE RUBBER GASKETED. LATERALS TO BE
WHITE IN COLOR AND MEET THE COVER REQUIREMENTS OF SECTION
5.06.07 CWS R&O 07-20.
4. ALL TESTING OF STORM SEWERS AND SANITARY SEWERS ARE TO BE IN
ACCORDANCE WITH C.W.S. REQUIREMENTS.
ENGINEER'S NOTE TO THE CONTRACTOR:
THE EXISTENCE AND LOCATION OF ANY UNDERGROUND UTILITIES OR
STRUCTURES SHOWN ON THESE PLANS ARE OBTAINED BY A SEARCH OF

(C.W.S.) STANDARD SPECIFICATIONS R & O 07-20. LATERALS TO BE
GREEN IN COLOR AND MEET THE COVER REQUIREMENTS OF SECTION
5.06.07 CWS R&O 07-20.
GENERAL NOTES (CONT.):
21. CONTRACTOR MUST VERIFY ALL EXISTING UTILITIES FOR BOTH
VERTICAL ELEVATION AND HORIZONTAL LOCATION PRIOR TO
START OF WORK (POTHOLE BEFORE DIGGING IF NECESSARY).

ROADWAYS AT THE END OF EACH DAYS WORK. NO TRENCH, ON
SITE OR OFF-SITE, SHALL BE LEFT AT ANY TIME IN AN UNSAFE
CONDITION. THE PERMIT HOLDER IS RESPONSIBLE FOR AND IS
LIABLE FOR HAZARDS OR DAMAGE RESULTING FROM THE
PROSECUTION OF THE WORK.
23. WORK PROVIDED FOR UNDER THE PERMIT SHALL INCLUDE
REPAIR OF EXISTING FACILITIES (ROADS, DITCHES, ETC.) AS MAY
BE NECESSARY, IN THE CITY INSPECTORS OPINION, TO
OVERCOME DETERIORATION OR DAMAGE WHICH OCCURRED IN

24. ONE AS-BUILT MYLAR DRAWING SHOWING ALL NEW PUBLIC
IMPROVEMENTS, INCLUDING ANY REVISION MADE TO THE
PREVIOUSLY APPROVED CONSTRUCTION PLANS AND, ALSO, ANY
IMPROVEMENT WHICH MAY IMPACT AN EXISTING PUBLIC
SYSTEM OR FACILITY, SHALL BE PROVIDED TO THE CITY BY A
REGISTERED CIVIL ENGINEER ALONG WITH AN ENGINEERS
CERTIFICATION OF INSTALLATION COMPLIANCE (FORM
ATTACHED). IN ADDITION, THE APPLICANT'S AS-BUILT DRAWINGS
SHALL BE TIED TO THE GPS NETWORK. THE APPLICANT'S
ENGINEER SHALL PROVIDE THE CITY WITH AN ELECTRONIC FILE
WITH POINTS FOR EACH STRUCTURE (MANHOLES, CATCH
BASINS, WATER VALVES, HYDRANTS AND OTHER WATER
SYSTEM FEATURES) IN TH DEVELOPMENT, AND THEIR
RESPECTIVE X AND Y STATE PLANE COORDINATES,
REFERENCED TO NAD 83 (91). INCLUDE ACROBAT (***.pdf)
FILE AND ONE 11x17 COPY OF AS-BUILTS.
25. A SEWER SYSTEM AIR-TEST AND (V.H.S.) TV TEST REPORT AND
ONE SET OF BLUELINE "AS-BUILTS" (OF EITHER OR BOTH THE
STORM AND SANITARY SEWER SYSTEMS) MAY BE REQUIRED BY
THE CITY FOR REVIEW AND APPROVAL PRIOR TO CONNECTION
OF ANY BUILDINGS TO THE SEWER SYSTEM.
26. THE PERMIT HOLDERS ENGINEER/INSPECTOR SHALL SUBMIT
DAILY INSPECTION REPORTS, ON A WEEKLY BASIS, TO THE
CITY'S INSPECTOR.
27. THE CITY'S INSPECTOR MAY, AT THEIR DISCRETION, REQUIRE
PROVISION OF TESTS AND OR REPORTS FROM THE PERMIT
HOLDER, PERMIT HOLDERS ENGINEER OR CONTRACTOR TO
VALIDATE CLAIMS OF MATERIAL OR CONSTRUCTION
ADEQUACY/COMPLIANCE. SUCH TESTS/REPORTS SHALL BE
PROVIDED AT THE PERMIT HOLDERS' EXPENSE.
28. THE PERMIT HOLDER SHALL PROVIDE A COPY OF A PROPERLY
EXECUTED RELEASE AND WAIVER DOCUMENT TO THE CITY FOR
EACH OWNERSHIP DISTURBED BY CONSTRUCTION ACTIVITY, AS
EVIDENCE OF DISTURBANCE RESOLUTION AND OWNER
SATISFACTION.
29. EXISTING MONUMENTS, PROPERTY CORNERS, AND SURVEY
MARKERS SHALL BE PROTECTED. REPLACEMENT SHALL BE AT
THE PERMIT HOLDERS' EXPENSE.
30. THE ENGINEER SHALL NOTIFY THE WASHINGTON COUNTY
SURVEYOR WHEN THE INITIAL AND FINAL LIFT OF ASPHALT HAS
BEEN PLACED.
31. THE PERMIT HOLDER SHALL PROVIDE TO THE CITY INSPECTOR,
IN WRITING, THE NAMES AND 24 HOUR EMERGENCY
TELEPHONE NUMBER OF TWO (2) PERSONS WHO HAVE
AUTHORITY TO RESOLVE PROBLEMS, TAKE CORRECTIVE ACTION
AND, IN GENERAL, WILL BE RESPONSIBLE IN CASE OF ANY
EMERGENCY. THE PERMIT HOLDER SHALL NOTIFY THE CITY
INSPECTOR, IN WRITING, OF ANY/ALL ASSIGNMENT CHANGES.
32. THE PERMIT HOLDER SHALL CAUSE HIS CONTRACTOR TO
PROVIDE TO THE CITY INSPECTOR, IN WRITING, THE NAME AND
24 HOUR EMERGENCY TELEPHONE NUMBER OF A DESIGNATED
"COMPETENT PERSON" RESPONSIBLE FOR CONSTRUCTION
SAFETY AS PER OR-OSHA, CHAP. 437, DIV. 3 CONSTRUCTION,
SUB-DIVISION P - EXCAVATIONS. THE CONTRACTOR SHALL
NOTIFY THE CITY INSPECTOR OF ANY/ALL ASSIGNMENT
CHANGES.
33. IT IS THE SOLE RESPONSIBILITY OF THE PERMIT HOLDER TO
PROVIDE FOR PROPER RIGHT-OF-ENTRY AND/OR EASEMENTS
PRIOR TO STARTING WORK. PROOF OF RIGHT-OF-ENTRY OR
PROPERLY EXECUTED EASEMENTS, SHALL BE PROVIDED TO
THE CITY. THE CITY SHALL IN NO WAY BE CONSTRUED TO BE
LIABLE FOR THE PERMIT HOLDERS FAILURE TO OBTAIN OR
PROVIDE FOR PROOF OF RIGHT-OF-ENTRY OR EASEMENTS.
34. BEFORE PLACEMENT OF ASPHALT, THE PERMIT HOLDER SHALL
CLEAN AND HAVE THE CITY COMPLETE A PRE-ACCEPTANCE
VIDEO INSPECTION OF ALL NEW SEWER LINES. THE PERMIT
HOLDER SHALL PROVIDE THE CITY WITH 30 DAYS OF NOTICE
FOR THE VIDEO INSPECTION. ANY DEFICIENCIES SHALL BE
REPAIRED BEFORE PLACEMENT OF FINAL LIFT.
35. (PRIVATE SYSTEMS) THE DEVELOPER SHALL MAINTAIN THE
WAER QUALITY FACILITY AND CORRECT ANY DEFECTIVE WORK,
AND SHALL PERFORM ALL SCHEDULED MAINTENANCE TO
ASSURE PROPER FUNCTIONING OF THE FACILITY.
WATER SUPPLY:
1. ALL WATER SYSTEM WORK AND MATERIALS SHALL COMPLY WITH THE
TUALATIN VALLEY WATER DISTRICT SPECIFICATIONS, THE OREGON
STATE HEALTH DIVISION ADMINISTRATIVE RULES CHAPTER 333 AND
THE 2008 ODOT/APWA STANDARDS, IN THAT ORDER.
2. ALL WATER LINE PIPE SHALL HAVE MINIMUM COVER OF 36 INCHES TO
THE FINISH GRADE.
3. THE CONTRACTOR SHALL TAKE PRECAUTIONS TO PROTECT PIPE
INTERIORS, FITTINGS AND VALVES AGAINST CONTAMINATION.
4. MAIN LINE VALVES THAT WILL NOT BE DISINFECTED BY CHLORINE IN
LINE FOR 24 HOURS SHALL HAVE THE INTERIORS SWABBED WITH A
5% HYPOCHLORITE SOLUTION BEFORE THEY ARE INSTALLED.
5. CONCRETE THRUST BLOCKS ARE TO BE CONSTRUCTED WHERE
INDICATED ON PLANS. THE BLOCK MINIMUM BEARING SURFACE
AGAINST UNDISTURBED SOIL IS SHOWN ON THE DETAIL SHEET.
THRUST BLOCKS SHALL BE ALLOWED TO CURE 14 DAYS BEFORE
PIPELINE PRESSURE TESTING.
6. THRUST AT FIRE HYDRANTS SHALL BE RESTRAINED USING MJ
RESTRAINED JOINT FOLLOWER GLANDS AS MANUFACTURED BY EBBA
IRON WORKS.
7. THE PIPELINE SHALL BE TESTED AT 150 PSI AT THE LOWEST POINT
AND HELD AT THAT PRESSURE FOR 2 HOURS. THE TEST IS TO BE
WITNESSED BY THE T.V.W.D. INSPECTOR AND THE ENGINEER.
8. THE PIPELINE IS TO BE THOROUGHLY DISINFECTED AND FLUSHED IN
ACCORDANCE WITH THE CURRENT T.V.W.D. SPECIFICATIONS.
9. PRIOR TO ENERGIZING THE WATER SYSTEM, A WATER SAMPLE SHALL
BE TAKEN BY THE T.V.W.D. INSPECTOR, SUBMITTED TO AN
ACCREDITED TESTING LAB, AND RETURNED TO T.V.W.D. INDICATING
NO HAZARD EXISTS.
10. ALL NEW DUCTILE IRON PIPE SHALL BE WRAPPED IN POLYTHYLENE
ENCASEMENT PER TVWD DET #2.
TVWD SHUTDOWN SEQUENCING NOTES:
1. CONTRACTOR TO POTHOLE AND VERIFY LOCATION, ELEVATION, AND
SIZE OF EXISTING WATER MAIN.
2. CONSTRUCT NEW 8" D.I. MAIN IN POLYETHYLENE ENCASEMENT (PER
TVWD ADDENDUM 3 AND DET 2). INSTALL 1" CARVS AT ALL
INTERMEDIATE HIGH POINTS (PER TVWD DET 601).
3. CONTRACTOR TO FLUSH, DISENFECT, AND TEST NEW 8" MAIN PRIOR
TO CONNECTION TO EXISTING MAIN. COORDINATE WITH TVWD FOR
INSPECTION AND TESTING.
4. UPON APPROVAL OF TESTING BY TVWD, COORDINATE WITH TVWD FOR
SHUTDOWN OF EXISTING MAIN. TVWD'S INSPECTOR WILL PROVIDE
NOTICE TO EFFECTED CUSTOMERS.
5. CONTRACTOR TO PROVIDE WRITTEN SEQUENCE SHUTDOWN PLAN TO
TVWD WITH A MINIMUM 2 WEEK NOTIFICATION PRIOR TO SHUTDOWN
OF EXISTING MAIN. CONTRACTOR IS LIMITED TO 4-HOUR
SHUTDOWN(S), TUESDAY THROUGH THURSDAY.
6. CONTRACTOR TO PROVIDE WRITTEN DE-WATERING PLAN FOR
EXISITNG PIPE AND TRENCH. EFFORTS SHALL BE MADE TO NOT
ALLOW WATER FROM DITCH TO ENTER PIPING. PROVIDE ADEQUATE
SUMP AND PUMPS AT CONNECTION POINTS. IT SHALL BE AT TVWD'S
SOLE DISCRETION TO ALLOW CONNECTION TO WATER SYSTEM.
7. CAP AND/OR PLUG SECTIONS OF EXISTING MAIN AT BOTH ENDS OF
ABANDONMENT. COORDINATE INSPECTION WITH TVWD. PIPE AND
FITTINGS SHALL BE SWABBED AND A VISUAL PRESSURE TEST WILL BE
CONDUCTED.
8. UPON ACCEPTANCE, COORDINATE WITH TVWD TO PUT MAIN IN
SERVICE AND TO FLUSH MAIN. TVWD TO TAKE WATER QUALITY
SAMPLE.
9. CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR OBTAINING
PASSING WATER QUALITY SAMPLES. ANY EFFORTS BEYOND
SAMPLING BY TVWD SHALL BE BILLED TO THE CONTRACTOR ON A
TIME AND MATERIALS BASIS AT TVWD'S DISCRETION.

DEMOLITION NOTES (CONT.):
16. CONTRACTOR SHALL COORDINATE WITH GOVERNING JURISDICTION FOR DISCONNECTION
OF EXISTING STORM DRAIN, SANITARY SEWER, FIRE, DOMESTIC WATER AND IRRIGATION
LINES CURRENTLY SERVING THE SITE IF REQUIRED. DEMOLISH, ABANDON, PLUG OR
REMOVE ALL UTILITIES IF INDICATED ON THE DRAWINGS.
17. CONTRACTOR SHALL BACKFILL ALL EXCAVATION RESULTING FROM, OR INCIDENTAL TO,
DEMOLITION ACTIVITIES. BACKFILL SHALL BE ACCOMPLISHED WITH APPROVED
MATERIALS AND SHALL BE PLACED AS COMPACTED ENGINEERED FILL AS REQUIRED TO
SUPPORT NEW IMPROVEMENTS. BEACKFILLING SHALL OCCUR IMMEDIATELY AFTER
DEMOLITION ACTIVITIES UNLESS OTHERWISE DIRECTED BY THE OWNER'S
REPRESENTATIVE.
18. DEMOLITION AREAS SHALL BE GRADED TO MATCH ADJACENT TERRAIN TO FORM A
SMOOTH, CLEAN (FREE OF DEBRIS), FREE DRAINING SURFACE UNLESS OTHERWISE
INDICATED ON THE DRAWINGS OR APPROVED BY THE OWNER'S REPRESENTATIVE.
19. CONTRACTOR SHALL COORDINATE WITH UTILITY COMPANIES AND OWNER'S
REPRESENTATIVE FOR RELOCATION OF EXISTING SIGNS, ETC. IF SHOWN ON THE
DRAWINGS. CONTRACTOR SHALL PROVIDE ALL MATERIALS AND PAY ALL COSTS FOR
EXCAVATION, BACKFILL, CONDUIT, WIRING, BASES AND APPURTENANCES REQUIRED TO
COMPLETE THE WORK.
20. ALL EXISTING WELLS, SEPTIC SYSTEMS OR DRAIN FIELDS SHALL BE DECOMMISSIONED
PER APPLICABLE CITY, COUNTY, STATE AND FEDERAL REQUIREMENTS.
DEMOLITION NOTES:
1. PRIOR TO STARTING DEMOLITION OPERATIONS THE CONTRACTOR SHALL APPLY FOR AND OBTAIN
ALL NECESSARY PERMITS REQUIRED BY FEDERAL, STATE AND LOCAL LAWS, CODES AND
REGULATIONS.
2. THE CONTRACTOR SHALL PROVIDE ALL THE "MEANS AND METHODS" NECESSARY TO PREVENT
MOVEMENT, SETTLEMENT OR COLLAPSE OF EXISTING STRUCTURES AND/OR IMPROVEMENTS TO
REMAIN, ON OR OFF SITE. THE CONTRACTOR IS RESPONSIBLE FOR ANY DAMAGE TO EXISTING
STRUCTURES AND/OR IMPROVEMENTS TO REMAIN AND SHALL RESTORE ANY DAMAGE TO THE
PRE-DEMOLITION CONDITION OR BETTER USING NEW MATERIALS ANY REPAIRS REQUIRED SHALL
BE PERFORMED AT THE CONTRACTOR'S SOLE COST AND EXPENSE. CONTRACTOR SHALL INSTALL
EROSION CONTROL MEASURES IN CONFORMANCE WITH THE EROSION CONTROL PLAN, NOTES
AND DETAILS AND TREE PROTECTION MEASURES PER PLAN AND ARBORISTS REPORT PRIOR TO
STARTING DEMOLITION OPERATIONS.
3. CONTRACTOR SHALL BE REQUIRED TO SECURE ANY NECESSARY PERMITS AND APPROVALS FOR
ALL OFF SITE MATERIAL SOURCES AND DISPOSAL FACILITIES. CONTRACTOR SHALL SUPPLY A
COPY OF APPROVALS TO OWNER'S REPRESENTATIVE PRIOR TO INITIATING WORK.
4. CONTRACTOR SHALL INSTALL EROSION CONTROL MEASURES IN CONFORMANCE WITH THE
EROSION CONTROL PLAN, NOTES AND DETAILS PRIOR TO STARTING DEMOLITION OPERATIONS.
5. THE LOCATION OF ALL UTILITIES SHALL BE MARKED IN THE FIELD PRIOR TO DEMOLITION.
CONTRACTOR SHALL PROTECT AND MAINTAIN IN A SAFE AND OPERABLE CONDITION ALL UTILITIES
INDICATED TO REMAIN AND PREVENT INTERRUPTION OF EXISTING UTILITY SERVICES EXCEPT
WHEN AUTHORIZED IN WRITING BY THE AUTHORITIES HAVING JURISDICTION. CONTRACTOR SHALL
PROVIDE TEMPORARY SERVICES, ACCEPTABLE TO GOVERNING AUTHORITIES AND OWNER'S
REPRESENTATIVE, FOR BUILDING(S) TO REMAIN AS REQUIRED DURING INTERRUPTIONS TO
EXISTING UTILITY SERVICES.
DEMOLITION NOTES (CONT.):
6. DEMOLITION ACTIVITIES AND EQUIPMENT SHALL NOT USE AREAS OUTSIDE THE PROJECT LIMITS
WITHOUT WRITTEN PERMISSION FROM THE OWNER'S REPRESENTATIVE AND/OR GOVERNMENTAL
AGENCIES HAVING JURISDICTION IF APPLICABLE.
7. THE CONTRACTOR SHALL KEEP ALL STREETS AND PUBLIC RIGHTS-OF-WAY CLEAN OF MUD, DIRT
AND DEMOLITION DEBRIS. THE CONTRACTOR SHALL MONITOR THE HAULING OF DEBRIS TO INSURE
THAT ANY SPILLAGE FROM TRUCKS IS PROMPTLY AND COMPLETELY REMOVED AND CLEANED UP.
IF REQUIRED AND/OR NECESSARY, THE CONTRACTOR SHALL COVER ALL HAUL VEHICLES.
8. CONTRACTOR SHALL CONDUCT DEMOLITION ACTIVITIES IN SUCH A MANNER TO ENSURE MINIMUM
INTERFERENCE WITH ROADS, STREETS, SIDEWALKS, WALKWAYS AND OTHER ADJACENT FACILITIES.
STREET CLOSURE PERMITS MUST BE RECEIVED FROM THE APPROPRIATE GOVERNMENTAL
AUTHORITY PRIOR TO THE COMMENCEMENT OF ANY ROAD OPENING OR DEMOLITION ACTIVITIES IN
OR ADJACENT TO THE RIGHT-OF-WAY.
9. CONTRACTOR SHALL CLEAN ADJACENT STRUCTURES AND IMPROVEMENTS OF DUST, DIRT AND
DEBRIS CAUSED BY DEMOLITION OPERATIONS AND RETURN ADJACENT AREAS TO CONDITIONS
EXISTING PRIOR TO THE START OF THE WORK.
10. DEBRIS SHALL NOT BE BURIED OR STOCKPILED ON THE PROPERTY. ALL DEMOLITION DEBRIS AND
MATERIALS SHALL BE DISPOSED OF AT OFFSITE LOCATIONS IN ACCORDANCE WITH ALL MUNICIPAL,
COUNTY STATE AND FEDERAL LAWS AND CODES AND REGULATIONS. THE CONTRACTOR SHALL
MAINTAIN RECORDS TO DEMONSTRATE PROPER DISPOSAL ACTIVITIES AND SHALL PROVIDE A COPY
TO THE OWNER OR OWNER'S REPRESENTATIVE UPON REQUEST. CONTRACTOR SHALL COORDINATE
AND PAY ALL COSTS INCLUDING DISPOSAL CHARGES.
DEMOLITION NOTES (CONT.):
11. PAVEMENT SHALL BE SAWCUT IN STRAIGHT LINES, AND EXCEPT FOR
BUTT JOINTS, SHALL EXTEND TO THE FULL DEPTH OF THE EXISTING
PAVEMENT. THE SAWCUT LINES SHOWN ON THE CONTRACT DRAWINGS
ARE SCHEMATIC AND NOT INTENDED TO SHOW THE EXACT ALIGNMENT
OF SUCH CUTS.
12. CONTRACTOR SHALL CONFORM TO APPLICABLE REGULATORY
PROCEDURES IF HAZARDOUS OR CONTAMINATED MATERIALS ARE
DISCOVERED.
13. CONTRACTOR IS RESPONSIBLE FOR JOB SAFETY AND SHALL PROVIDE,
ERECT AND MAINTAIN TEMPORARY BARRIERS, WARNING SIGNS,
TRAFFIC CONES AND PROTECTION AS NECESSARY DURING DEMOLITION
ACTIVITIES.
14. CONTRACTOR SHALL REMOVE ALL EXISTING STRUCTURES, FACILITIES,
IMPROVEMENTS, ETC. AS SHOWN UNLESS OTHERWISE DIRECTED BY
THE OWNER OR OWNER'S REPRESENTATIVE.
15. CONTRACTOR SHALL DISCONNECT, ABANDON, PLUG OR REMOVE ALL
POWER, TELEPHONE, TV AND GAS UTILITIES SERVING THE EXISTING

TREE PLANTING NOTES
1. PRIOR TO STREET TREE PLANTING ALONG, LANDSCAPE
CONTRACTOR SHALL EXCAVATE TO A DEPTH OF 36-INCHES
AND BACKFILL WITH AMENDED TOPSOIL. TOPSOIL AMENDMENTS
SHALL BE DETERMINED BY THE LANDSCAPE CONTRACTOR.
2. IRRIGATION TO BE DESIGN/BUILD BY THE LANDSCAPE
CONTRACTOR.
3. NEW TREES THAT ARE PLANTED TO MEET THE EFFECTIVE CANOPY
REQUIREMENTS SHALL CONFORM TO THE APPLICABLE
STANDARDS IN THE TIGARD URBAN FORESTRY MANUAL. THEY
SHALL BE PLANTED IN ACCORDANCE WITH THE AMERICAN
NATIONAL STANDARDS INSTITUTE (ANSI) STANDARDS FOR TREE
PLANTING (A300, PART 6) AND ADDITIONAL STANDARDS
ADOPTED BY THE OREGON LANDSCAPE CONTRACTORS BOARD.
NURSERY STOCK SHALL MEET THE REQUIREMENTS OF THE
AMERICAN ASSOCIATION OF NURSERYMEN FOR NURSERY STOCK
(ANSI Z60.1) FOR GRADE NO.1 OR BETTER. DOUBLE STAKE TREES
IF NEEDED FOR STABILITY DURING THE ESTABLISHMENT PERIOD.
4. ROOT BARRIERS SHALL BE INSTALLED ACCORDING TO THE
MANUFACTURER'S SPECIFICATIONS WHEN A STREET TREE IS
PLANTED WITHIN 5' OF ANY HARD SURFACE, PAVING, OR UTILITY
BOX.
