Please complete the attached form and email it back to us. I have copied Tom Hochstatter on this email to see if he will require revised plans or other information regarding this reduction of project valuation from $5,968,624 to$4,600,000. Tom will get back to you on Monday. Once we have the revised application and Tom's decision regarding plans we can adjust the permit fees and provide a new invoice for this permit. Thank you.
