However, when we received the revised plumbing application that was submitted to us on May 8,the Commercial Fixture Work section on the second page only included the capped fixtures and not the "added" fixtures for the new school. We will need this section to be completed to assess the sewer fees. If you can revise the second page of the application and get this to me this week, I should be 1 able to complete your permit before I go on vacation. I will be out of the office all of next week. Regarding the building fixtures plumbing permit(interior plumbing), this permit is currently being reviewed by our plumbing inspector, and I previously requested a new building fixtures plumbing permit application since the fixture counts listed in the Commercial Fixture Work section on the second page did not match the fixture counts on the front of the application. Please see my original email listed below. We received a revised permit application for the site utilities. However, we never received the revised building fixtures plumbing permit application. The sewer SDC fees are based on the fixture counts listed in the Commercial Fixture Work section of the application that tells us which fixtures will be capped, added, or replaced. After reviewing all of the permit applications, each description of work section says, "Construction of new 2-story, 80,000 SF Elementary School Building." On each application,we need a detailed description of work that accurately reflects the work that is taking place. We would appreciate it If you can complete new applications for the building permit, site work permit, demolition permit,and plumbing permits with a more detailed description of work. Also, regarding the site utilities plumbing permit, we received one plumbing 3 application for the site utilities and plumbing fixtures. We have separate plumbing permit applications for each, and we would appreciate it if you can separate the site utilities scope of work from the plumbing fixture scope of work by completing separate plumbing applications that I have attached above. When you submit the plumbing fixture application, please complete the Commercial Fixture Work section on the second page. We will need to know if you are adding or relocating fixtures in addition to capping them. I noticed the number of capped sinks on the second page of the plumbing application do not match the number of sinks on the front page. Please confirm the fixture counts before submitting the new applications. Lastly, after reviewing the demolition plan, it appears a Twality Middle School portable classroom will be relocated. In this case,we will need a Commercial Manufactured Structure permit for the setup of the structure, and all of the other permits associated with the work when applicable. I have included additional information below to help you: Here is a list of permits that may be required for relocating the portable building: • Commercial Manufactured Structure (CMS) Permit: This permit is required for the setup of the structure and includes the concrete slab or foundation, and electrical feeder, water and sewer connections not exceeding 30 lineal feet outside the structure. Erosion control is also collected on this permit. Permit fees include setup, state administration and 12%surcharge for a total of$338.56. Permit fees for erosion control are based on the project valuation. Submit (3) copies of a site plan and construction plans(see SFM application attached for requirements). • Building Permit(BUP): This permit is required for any exterior work such as accessibility, covered patios, decks, etc. and any interior modifications to the structure. Permit fees are based on the project valuation. Submit(3) copies of site plan and construction plans. • Plumbing Permit(PLM): This permit is required for site utilities beyond the 30 lineal feet under the CMS permit and to determine sewer SDC fees based on the number of plumbing fixtures in the structure. • Mechanical Permit(MEC): This permit may be required if add A/C to exterior of the structure. • Electrical Permit(ELC): This permit may be required if altering the branch circuits for interior modifications of the structure.
