Per Chapter 17, Section 1704.1.2 of the International Building Code (IBC), we have performed special inspection of the
following item(s) per our inspection reports only:
Expansion Anchors
The non-compliance (NC) items recorded for each of the above inspection categories, as listed on the non-compliance
report, have been either repaired and re-inspected by CTI for compliance or reviewed and accepted by the engineer of
record for the project in the"As-Built Condition" or through an RFI.
The issuance of this final report is based on the engineer's acceptance of the NC items not specifically re-inspected by

All requested inspections and tests were performed and reported according to the requirements of project documents and
to the best of our knowledge, the work was in conformance with the approved plans and specifications, approved change
orders and applicable workmanship, provisions of the State Building Code and Standards, as well as the structural
engineer's design changes, approvals and verbal instructions.
Our construction inspection and testing services were provided and intended to help reduce the risk of construction
defects, deficiencies or omissions that arose during and after construction. Our services do not constitute a warranty or
guarantee of any type. Even with diligent construction monitoring and testing, construction defects, deficiencies or
omissions in the contractor's work may exist. It is the contractor's responsibility to perform their work in accordance with
the approved construction documents.
CTI has provided its professional services with the degree of care and skill currently exercised under similar
circumstances by members of its profession in the same locale. Statements made in our reports are based on technical
judgments and should not be construed to be conclusive representations of fact. Our reports pertain to the material
tested/inspected only.

Re: Trampoline Anchor Testing Recommendations

Dear Cody,
At your request, WDY has prepared the following recommendations for testing the trampoline concrete
anchors at the subject site. We understand that anchors were installed without special inspection and
some anchors did not achieve full embedment. Our recommendations are based on 1.5 times the cracked
concrete design capacity of the 3/8" diameter x 3" Titen HD anchors noted on the project drawings,
assuming a minimum 5" concrete slab on grade with a minimum 2,500 psi 28 day concrete compressive
strength (modified to allowable strength levels). Anchor testing shall be performed by an independent
testing agency retained by you or your company.
Testing Recommendations:
Tension test load: 1,250 pounds. Acceptance criteria—anchor maintains test load for 1 minute.
1. Test all anchors that have had the top of the anchor cut off or anchor length above the slab that
would indicate incomplete embedment.
2. Test 10% of remaining anchors. Notify engineer after first failed anchor in this group and load at
failure before continuing.
3. If any of the 10%fail—test an additional 10%.
4. If any of the second 10%fail—test all remaining anchors.
5. Prepare a report of findings, including identifying anchors tested, anchors that fail the test (if any)
and load at failure.
Limitations:
WDY has not analyzed the trampoline system. The recommendations contained herein are based solely
on the capacity of the anchors specified on the project drawings dated Rev no. 1, 02.10.2017. WDY is not
responsible for the design of the trampoline system or trampoline anchorage.
