We received the applications and plans you sent in the mail for Firenza Pizza located at 12280 SW Scholls Ferry
Rd. However, we did not receive an "authorized signature"at the bottom of the mechanical permit application. I hav
attached the application above for you to sign and return to us. Also, for future reference, we received one set of e
mechanical plans to review, and we require (2)sets. Our commercial plans examiner, Dan Nelson,said he will pull a se
of mechanical pages from the building permit plans to add to the mechanical permit. The electrical permit application

also requires a supervising electrician's signature and license information prior to issuing the permit. We can make the
electrical permit ready to issue now for you, or you can have your electrical contractor pull the permit online before
do the work. Regarding the plumbing permit, since this work does not require plan review, I can make this permit read they
to issue. Would you like us to make the electrical permit ready to issue, or would you rather have the contractor pull
